根除幽门螺杆菌对老年功能性消化不良的治疗作用

Therapeutic effect of Helicobacter pylori eradication on functional dyspepsia in elderlies

目的探讨根除幽门螺杆菌(H.pylori)对老年功能性消化不良(functional dyspepsia,FD)的治疗效果。方法选取2016年3月至2018年4月我院诊治的92例老年FD合并H.pylori阳性患者进行研究,采用随机分组表分为常规组(常规促进胃动力治疗,46例)和干预组(常规治疗联合H.pylori根除治疗,46例),比较两组患者治疗前后症状评分、疗效及药物不良反应发生率。结果治疗前,常规组和干预组患者上腹不适、饱胀感、灼烧感评分比较差异无统计学意义(均P>0.05)。治疗14 d,两组患者上腹不适、饱胀感、灼烧感评分均下降,且干预组患者下降程度更大(均P<0.05)。干预组患者治疗总有效率显著高于常规组(95.45%vs 76.19%;χ~2=5.1342,P=0.0235)。两组患者治疗14 d期间,口干、恶心呕吐、头晕、皮疹、乏力发生率比较差异无统计学意义(均P>0.05)。结论对老年FD合并H.pylori阳性患者实施H.pylori根除性治疗能协助改善患者消化不良症状,提高患者疗效,同时不增加药物不良反应发生率。

Objective To observe the effect of H.pylori eradication on functional dyspepsia(FD)in elderly patients.Methods Ninety-two H.pylori positive elderly patients with FD who were treated in our hospital during Mar 2016 to Apr 2018 were enrolled and divided into the conventional treatment group(conventional acid suppression,promotion of gastric motility therapy,46 cases)or the intervention group(conventional treatment combined with H.pylori radical treatment,46 cases)randomly.The symptom scores,efficacy and incidence of adverse drug reactions were compared between the two groups before and after treatment.Results Before treatment,there were no statistically significant differences in upper abdominal discomfort,satiety and burning sensation between the two groups(all P>0.05).After treatment for 14 days,the incidence rates of upper abdominal discomfort,satiety and burning sensation decreased in both groups,and the degree of decline in the intervention group was significantly higher(all P<0.05).The total efficiency rate in the intervention group was significantly higher than that in the conventional treatment group(95.45%vs 76.19%,χ~2=5.1342,P=0.0235).There were no significant differences in the incidences of dry mouth,nausea,vomiting,dizziness,rash and fatigue between the two groups(all P>0.05).Conclusion In elderly H.pylori-positive patients with FD,the implementation of H.pylori eradication therapy can help improve the symptoms of dyspepsia and improve the efficacy without increasing the incidence of adverse drug reactions.
