胱抑素C、视黄醇结合蛋白、游离脂肪酸与妊娠期糖尿病发生发展的相关性研究 被引量:4

Correlation between cystatin C,retinol binding protein,free fatty acid and development of gestational diabetes.

摘要 目的探讨胱抑素C(Cys-C)、视黄醇结合蛋白(RBP)、游离脂肪酸(FFA)与妊娠期糖尿病发生发展的相关性,以指导临床早期诊治。方法将2017年5月-2019年4月本院收治的85例妊娠期糖尿病孕妇作为观察组,另将同期于本院接受产检的90例正常孕妇作为健康对照组。通过葡萄糖氧化酶法检测2组孕妇的血糖水平,并经免疫比浊法、酶法对2组孕妇的血清Cys-C、RBP、FFA表达实施测定;利用Spearman秩相关性分析法分析血清Cys-C、RBP、FFA与妊娠期糖尿病孕妇血糖水平相关性。结果观察组FPG、1 h PG、2 h PG水平均高于健康对照组,差异有统计学意义(P <0.05);观察组血清Cys-C、RBP、FFA表达均高于健康对照组,差异有统计学意义(P <0.05);经Spearman秩相关性分析发现:妊娠期糖尿病孕妇的血清Cys-C、RBP、FFA表达与血糖水平呈正相关(r值分别为0.746、0.684、0.663,P <0.05);且血清Cys-C表达与RBP、FFA表达均呈正相关(r值分别为0.594、0.509,P <0.05);血清RBP表达与FFA表达呈正相关(r=0.337,P <0.05)。结论胱抑素C、视黄醇结合蛋白、游离脂肪酸与妊娠期糖尿病发生发展存在明显相关性,监测孕妇血清胱抑素C、视黄醇结合蛋白、游离脂肪酸表达,能为临床诊断妊娠期糖尿病和评估病情提供信息。 Objective To investigate the correlation between the development of cystatin C( Cys-C),retinol binding protein( RBP) and free fatty acid( FFA) and gestational diabetes,in order to guide the early diagnosis and treatment. Methods A total of 85 pregnant women with gestational diabetes admitted to our hospital from May 2017 to April 2019 were used as observation groups. In addition,90 normal pregnant women who underwent medical examination at the same time were used as the controls. The blood glucose levels of the two groups of pregnant women were detected by glucose oxidase method,and the expressions of serum Cys-C,RBP and FFA were measured by immunoturbidimetry and enzymatic methods. Spearman rank correlation analysis was used to analyze the correlation between serum Cys-C,RBP,FFA and blood glucose levels in pregnant women with gestational diabetes. Results The FPG,1 hPG and 2 hPG levels of the pregnant women in the observation group were higher than those in the control group( P < 0. 05). The levels of serum Cys-C,RBP and FFA in the observation group were higher than those in the control group( P < 0. 05). According to Spearman rank correlation analysis,serum Cys-C,RBP,FFA expression and blood glucose levels were positively correlated in pregnant women with gestational diabetes( r =0. 746,r = 0. 684,r = 0. 663,P < 0. 05). The serum Cys-C expression was positively correlated with RBP and FFA expression( r = 0. 594,r = 0. 509,P < 0. 05). Serum RBP expression was positively correlated with FFA expression( r = 0. 337,P <0. 05). Conclusion Cystatin C,retinol-binding protein and free fatty acid have obvious correlation with the development of gestational diabetes. Monitoring the expression of serum cystatin C,retinol-binding protein and free fatty acid in pregnant women can be used for clinical diagnosis of gestational diabetes,and provide information to assess the condition.
