GM、G试验对机械通气并发肺曲霉菌感染的诊断价值 被引量:2

The Diagnostic Value of GM and G Tests for Mechanical Ventilation Complicated by Pulmonary Aspergillus Infection

摘要 探讨肺泡灌洗液(BALF)GM试验和血清GM、G试验对肺曲霉菌感染的诊断价值。检测莆田学院附属医院200例机械通气的患者,分为侵袭性肺曲霉菌病(IPA)组及非IPA组(拟诊病例不纳入分组)。结果:IPA组BALF血清GM水平高于非IPA组血清GM水平;IPA组中,BALF GM、血清GM、血清G试验的敏感性、特异性、PPV、NPV、符合率低于联合检测相应指标;血清G试验的敏感性、特异性低于BALF GM、血清GM试验相应指标;血清GM试验的敏感性、特异性低于BALF GM试验相应指标。结果表明:在诊断IPA中,血清G试验具有不确定性;BALF GM、血清GM试验可提供早期诊断依据;BALF GM试验优于血清GM试验。 We investigate the diagnostic value of the BALF GM test and serum GM and G tests for pulmonary aspergillus infection.200 patients treated by mechanical ventilation in the Affiliated Hospital of Putian University are tested and divided into invasive pulmonary aspergillosis(IPA)group and non-IPA group,excluding the patients to be diagnosed.We find that the serum GM level of BALF in the IPA group is higher than that in the non-IPA group.In the IPA group,the sensitivity,specificity,PPV,NPV and coincidence rate of the BALF GM,serum GM and serum G tests are lower than those of the combined tests.The sensitivity and specificity of the serum G test are lower than those of the BALF GM and serum GM tests,and the sensitivity and specificity of the serum GM test are lower than those of the BALF GM test.The results show that the serum G test is uncertain in the diagnosis of IPA and that the BALF GM and serum GM tests can provide evidence for early diagnosis with the BALF GM test superior to the serum GM test.
