社区卫生定向服务模式在社区高血压和糖尿病管理中的应用探讨

目的:探讨社区卫生定向服务模式在社区高血压和糖尿病管理中的应用效果。方法:选取2017年7月-2019年7月社区高血压和糖尿病患者280例,随机分为两组。对照组进行常规护理;研究组进行社区卫生定向服务模式。比较两组患者相关疾病知识了解情况、血压及血糖水平、护理满意度。结果:研究组相关疾病知识了解情况优于对照组,差异有统计学意义(P<0.05);研究组舒张压、收缩压及空腹血糖、随机血糖、糖化血红蛋白水平均低于对照组,差异均有统计学意义(P<0.05);研究组护理满意度高于对照组,差异有统计学意义(P<0.05)。结论:社区高血压和糖尿病患者管理中,社区卫生定向服务模式的效果显著。

Objective:To explore the application effect of community health oriented service model in the management of hypertension and diabetes mellitus in the community.Methods:From July 2017 to July 2019,280 patients with hypertension and diabetes mellitus in the community were selected and they were randomly divided into two groups.The control group received conventional nursing.The research group carried out community health oriented service model.The knowledge of related diseases,blood pressure and blood glucose levels,and nursing satisfaction were compared between the two groups.Results:The knowledge of related diseases in the study group was better than the control group,with the difference was statistically significant(P<0.05).The diastolic blood pressure,systolic blood pressure and fasting blood glucose levels,random blood glucose levels,glycosylated hemoglobin levels of the study group were lower than the control group(P<0.05).Nursing satisfaction in study group was higher than the control group,with the difference was statistically significant(P<0.05).Conclusion:In the management of patients with hypertension and diabetes mellitus in community,the community health oriented service model has a significant effect.
