超声引导腕部神经阻滞在断指再植术中的应用

Application of Ultrasound-Guided Wrist Nerve Block in Replantation of Severed Finger

目的探讨超声引导下腕部神经阻滞在断肢再植中的应用效果。方法筛选50例进行断指再植术患者作为研究群体,按照麻醉方法进行分组,使用常规单次臂丛神经阻滞的患者设定为对照组,超声引导下腕部神经阻滞的患者设定为观察组。对术前、术后12 h、术后24 h、术后48 h四个阶段的临床VAS评分情况进行对比。结果术前两组患者VAS评分没有明显差异(P>0.05)。手术治疗之后,在12 h、24 h和48 h三个阶段,观察组患者的VAS评分明显低于对照组(P<0.05),说明超声引导下腕部神经阻滞的临床干预效果更好。结论对断指再植患者来说,超声引导下腕部神经阻滞效果较好,成功率高。在临床应用中可结合实际情况具体选择相关方法及有关设备。

Objective To investigate the effect of ultrasound-guided wrist nerve block in replantation of severed limb.Methods A total of 50 patients undergoing finger replantation were selected as the subjects,and were grouped according to the anesthesia method.The patients with conventional single brachial plexus block were set as the control group,and the patients with ultrasoundguided wrist block were set as the observation group.The clinical VAS scores at the four stages of preoperative,postoperative 12 h,postoperative 24 h and postoperative 48 h were compared between two groups.Results There was no significant difference in VAS score between the two groups before surgery(P>0.05).After surgical treatment,VAS scores of patients in the observation group were significantly lower than those in the control group at 12,24 and 48 h(P<0.05),indicating better clinical intervention effect of ultrasound-guided wrist nerve block.Conclusion For patients with replantation of amputated finger,the effect of wrist nerve block guided by ultrasound is better and the success rate is high.In clinical application,relevant methods and equipment can be selected according to the actual situation.
