Lymph node management in cN0 supraglottic laryngeal carcinoma:a Meta-analysis

目的系统评价cN0声门上型喉癌颈部淋巴结处理方式。方法采用计算机检索万方数据库、中国知网、PubMed及Embase,按纳入与排除标准选择文献,根据MINORS评价条目评价文献质量,提取数据进行定性描述,采用Stata/SE 15.1软件进行Meta分析,使用I2、P评估异质性,采用OR作为结局变量进行汇总分析。结果共纳入7篇文献共883例病例。在cN0声门上型喉癌颈部淋巴结的处理方法中,颈部清扫组、放射治疗组、综合治疗组之间相比较,5年颈部淋巴结复发转移率和5年总生存率无明显差异(P>0.05),但均优于临床观察组(P<0.05)。结论术者通过严格的术前评估,结合临床诊断,需对cN0声门上型喉癌颈部淋巴结进行处理。

Objective To evalnate the lymph node management of cN0 supraglottic laryngeal carcinoma systemicly.Methods The Wanfang,CNKI,PubMed and Embase were searched,and literatures were selected according to the inclusion and exclusion criteria.Literature quality was evaluated according to MINORS evaluation entries.Stata/SE 15.1 software was used for Meta-analysis.Heterogeneity was assessed using I2 and P,and OR was used as outcome variables for summary analysis.Results There were 7 articles including 883 cases enrolled.In the treatment of cervical lymph nodes of cN0 supraglottic laryngeal carcinoma,there was no significant difference in 5-year recurrence and metastasis rate,5-year overall survival rate among the neck dissection treatment group,the neck radiotherapy group,and the comprehensive treatment group(P>0.05),but all of them were superior clinical observation(P<0.05).Conclusion Through strict preoperative evaluation and clinical diagnosis,it`s needed for the operator to deal with cervical lymph nodes of cN0 supraglottic laryngeal carcinoma.
