EBV-DNA载量在传染性单核细胞增多症患儿中的变化情况及诊断价值 被引量:14

摘要 目的研究EB病毒(EBV)-DNA载量在传染性单核细胞增多症(IM)患儿中的变化情况及诊断价值。方法选取该院2017-2019年收治的IM患儿63例为研究组,另选取感染EBV的非IM患儿113例为对照组。比较研究组与对照组EBV-DNA载量、血常规、生化指标水平;比较研究组不同EBV-DNA载量IM患儿的血常规、生化指标水平;分析EBV-DNA载量对IM的诊断价值。结果研究组白细胞计数、淋巴细胞绝对值、单核细胞绝对值、嗜碱性粒细胞绝对值、EBV-DNA载量、丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、乳酸脱氢酶、α-羟基丁酸脱氢酶、腺苷脱氨酶水平高于对照组,而血小板计数、中性粒细胞绝对值、嗜酸性粒细胞绝对值低于对照组,差异均有统计学意义(P<0.05)。低、中、高载量IM患儿血常规和生化指标水平比较,差异均无统计学意义(P>0.05)。受试者工作特征曲线分析结果显示,EBV-DNA载量诊断IM的曲线下面积为0.873,灵敏度为0.794,特异度为0.858。结论IM患儿EBV-DNA载量高于感染EBV的非IM患儿,且IM患儿的肝功能及心功能损伤程度更重。EBV-DNA载量在诊断IM中具有较高的临床应用价值。 Objective To study the change and diagnostic value of EB virus(EBV)-DNA load in children with infectious mononucleosis(IM).Methods A total of 63 children with IM admitted to the hospital from 2017 to 2019 were selected as the study group,and 113 non-IM children with EBV infection were selected as the control group.Compared the EBV-DNA load,blood routine and biochemical indexes levels between the study group and the control group.Compared the blood routine and biochemical indexes levels of IM children with different EBV-DNA loads in the study group.Analyzed the diagnostic value of EBV-DNA load for IM.Results The white blood cell count,lymphocyte absolute value,monocyte absolute value,basophil absolute value,EBV-DNA load,alanine aminotransferase,aspartate aminotransferase,lactate dehydrogenase,α-hydroxy-butyrate dehydrogenase and adenosine deaminase levels in the study group were higher than those in the control group,while the platelet count,neutrophil absolute value and eosinophil absolute value in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in blood routine and biochemical indexes levels among low,medium and high load IM children(P>0.05).The results of receiver operating characteristic curve showed that the area under the curve of EBV-DNA load in the diagnosis of IM was 0.873,the sensitivity was 0.794,and the specificity was 0.858.Conclusion The EBV-DNA load in IM children is higher than that in non-IM children with EBV infection,the liver function and heart function damage of IM children are more severe.EBV-DNA load has high clinical application value in the diagnosis of IM.
