阿戈美拉汀对抑郁症患者睡眠质量及血清褪黑素水平的影响

Effect of agomelatine on sleep quality and serum melatonin of patients with depression

目的:探讨阿戈美拉汀对抑郁症患者睡眠质量及血清褪黑素(MT)水平的影响。方法:100例抑郁症患者随机分为研究组与对照组;研究组单用阿戈美拉汀片,对照组单用舍曲林分散片;疗程8周。治疗前及治疗2、4、8周进行汉密尔顿抑郁量表(HAMD)、匹兹堡睡眠指数量表(PSQI)评估,检测血清MT水平,记录不良反应。结果:研究组44例、对照组46例完成观察。治疗前两组间HAMD、PSQI评分及血清MT水平比较差异无统计学意义;治疗后两组HAMD及PSQI评分较治疗前明显减少(P均<0.01),但各时间点两组间HAMD评分差异无统计学意义;治疗4及8周后研究组PSQI评分明显低于对照组(P均<0.01);治疗后各时间点研究组血清MT水平明显高于对照组(P<0.05或P<0.01)。研究组不良反应发生率与对照组比较差异无统计学意义。研究组血清MT水平与HAMD及PSQI评分呈负相关(P均<0.05)。结论:阿戈美拉汀具有明显改善抑郁症患者睡眠障碍作用,这与其提高血清MT水平具有相关性。

Objective:To explore the effect of agomelatine on sleep quality and serum melatonin(MT)of patients with depression.Method:100 patients with depression were randonly divided into experimental group and control group.Patients in experimantal group were administered with agomelatine,while those in control group were administered with sertraline.The course of treatment lasted 8 weeks.Hamilton Depression Scale(HAMD)and Pittsburgh Sleep Index(PSQI)were adminstered before treatment and the 2nd,4th,8th weekend after treatment.The serum MT was measured and the adverse reactions were recorded meanwhile.Results:44 patients in experimental group and 46 in control group accomplished the treatment.There was no statistical difference in scores of HAMD,PSQI and serum level of MT between two groups.Scores of HAMD and PSQI after treatment in both groups were lower than those before treatmen(all P<0.01).However,the HAMD scores between the two groups had no statistical differences at any time point.The PSQI score of experimental group was significantly lower than that of control group at the 4th,8th weekend after treatment(all P<0.01).The serum level of MT in experimental group was higher than that of the control group(P<0.05 or P<0.01)at every time-point after treatment.There's no statistical difference in incidence rate of adverse reactions between the experimental group and the control group.There's a negative correlation between serum level of MT and scores of HAMD and PSQI.Conclusion:Agomelatine could improve sleep quality of patients with depression,which correlated to the increase ofserum MT level.
