Sparkle map标测技术应用于室上性心动过速射频消融治疗的效果分析

摘要 目的探讨Sparkle map标测技术应用于室上性心动过速射频消融治疗的效果。方法选择2013年6月至2018年9月间我院126例采用射频消融治疗室上性心动过速的患者,根据治疗技术的不同分为对照组与试验组,对照组采用传统标测技术,试验组采用Sparkle map标测技术。比较两组患者手术时间、标测消融时间、总曝光时间、总曝光剂量、消融能量、标测消融“零射线”例数的差异,以及两组患者治疗前后心功能各项指标的差异。结果试验组(70.52±18.53)min手术时间短于对照组(78.19±20.46)min、试验组(24.11±9.30)min标测消融时间短于对照组(31.08±14.62)min、试验组(7.63±4.57)min总曝光时间短于对照组(16.59±8.35)min、试验组(7.63±4.57)minmGy总曝光剂量少于对照组(186.20±109.75)Gy、试验组(965.13±728.62)μGym^2总DAP少于对照组(2357.21±2041.95)μGym^2,差异均有统计学意义(P<0.05);试验组患者消融能量(32.96±4.28)周、标测消融“零射线”例数显著高于对照组患者(31.18±4.53)周,组间的差异均有统计学意义(P<0.05);治疗后进行组间比较,试验组患者射血分数(LVEF)高于对照组(46.79±4.07)%,差异有统计学意义(P<0.05);试验组并发症发生率为0.00%,低于对照组(9.52%),组间的差异均有统计学意义(P<0.05)。结论Sparkle map标测技术弥补了传统标测技术的缺陷,可以记录完整心腔内局部心肌激动的双极信号及其电压激动图,显示心肌组织激动产生的电压信号,有助于确定室上性心动过速的主导折返环路径。 Objective To explore the effect of Sparkle map mapping technology in radiofrequency ablation treatment of supraventricular tachycardia.Methods From June 2013 to September 2018,126 patients with supraventricular tachycardia treated by radiofrequency ablation in our hospital were studied.According to the different treatment techniques,they were divided into control group and experimental group.The control group used traditional mapping Technology,the test group uses Sparkle map mapping technology.Compare the statistical differences of the two groups of patients in the operation time,the time of mapping ablation,the total exposure time,the total exposure dose,etc.,compare the statistical differences of the two groups of patients in the ablation energy and the number of“zero-ray”cases of ablation,compare the two groups The statistical difference of various indexes of heart function before and after treatment.Results The operation time of the experimental group(70.52±18.53)min was shorter than that of the control group(78.19±20.46)min,the experimental group(24.11±9.30)min was shorter than the control group(31.08±14.62)min,and the experimental group(7.63±4.57)min total exposure time is shorter than the control group(16.59±8.35)min,the experimental group(7.63±4.57)minmGy total exposure dose is less than the control group(186.20±109.75)Gy,the experimental group(965.13±728.62)μGym^2 total DAP is less In the control group(2357.21±2041.95)μGym^2,the differences were statistically significant(P<0.05);in the experimental group,the ablation energy(32.96±4.28)weeks and the number of"zero-ray"cases of ablation in the experimental group were significantly higher than those in the control group(31.18±4.53)weeks,the differences between the groups were statistically significant(P<0.05);after treatment,the LVEF of patients in the experimental group was higher than that of the control group(46.79±4.07)%.There was statistical significance(P<0.05);the complication rate in the experimental group was 0.00%,which was lower than that in the control group(9.52%),and the differences between the groups were statistically significant(P<0.05).Conclusion Sparkle map mapping technology makes up for the shortcomings of traditional mapping technology.It can record the bipolar signal of local myocardial activation in the complete heart cavity and its voltage activation map,display the voltage signal generated by the activation of myocardial tissue,and help determine supraventricular sex,The dominant reentrant path of tachycardia.
