摘要
胰腺纤维化是慢性胰腺炎的典型病理特征,在中医学中虽没有慢性胰腺炎胰腺纤维化的病名和专门论述,但根据其临床常见表现可将其纳入中医内科学中的“呕吐”“痞证”“胃脘痛”“腹痛”“泄泻”“癥瘕积聚”等疾病范畴。胰无论在解剖生理方面,还是在病理表现方面,与脾关系密切,可将此种关系归纳为“脾胰同源”。基于“脾胰同源”理论,慢性胰腺炎胰腺纤维化的主要病机为脾虚失运,痰湿内停,治疗时应以脾为中心,健脾祛湿为主,兼以疏肝补肾,辅以活血通络。
Pancreatic fibrosis is the characteristic histopathology of chronic pancreatitis.There is no disease name and professional discussion on the chronic pancreatitis in TCM,but the chronic pancreatitis belongs to vomiting disease,epigastric fullness syndrome,epigastralgia,abdominal pain,diarrhea,concretions and gatherings,etc.in internal medicine of TCM.Pancreas has a close relationship on the anatomicophysiology and pathological manifestations with spleen,which can be recognized as homogeny of pancreas and spleen.Spleen difficiency and phlegm-dampness distension is the main pathogenesis of pancreas fibrosis based on the theory of homogeny of pancreas and spleen,so the treatment should focus on the spleen for tonifying spleen and eliminating phlegm-dampness and use the methods of soothing the liver,reinforcing kidney and promoting blood circulation for removing obstruction in collaterals.
