1p/19q、MGMT启动子甲基化和VEGF对不同级别胶质瘤患者的影响

摘要 目的探讨IDH1、1p/19q、MGMT启动子甲基化、VEGF对不同级别胶质瘤患者的影响。方法筛选了星形细胞瘤和少突胶质细胞肿瘤及胶质母细胞瘤患者30例,其中高、低级别各15例(低级别组:A组;高级别组:B组)。记录患者的一般情况、出现肿瘤相关功能缺失的临床症状、体征、术前术后影像学特点;记录的患者分子病理IDH1、1p/19q、MGMT启动子甲基化、VEGF的情况;根据分子病理结果和高危因素对低级别组进行分层,分为低危组和高危组,对高危组(C组)和高级别组(B组)采取术后7天早期口服替莫唑胺(75mg/kg/m^2,连续5天)化疗方案,记录患者相关药物不良反应情况。应用SPSS统计软件进行数据的统计学分析。P<0.05表示差异有统计学意义。结果A和B组患者一般情况,KPS评分A组要高于B组(P<0.05)。两组患者肿瘤发生部位,额叶、颞叶的比例高于额颞、顶叶及小脑(P<0.05),额颞比例高于顶叶、小脑(P<0.05);患者术前功能缺失的患者明显高于无功能缺失者(P<0.01)。≤40岁、IDH突变型及分子病理IDH突变型及VEGF阴性的患者B组低于A组(P<0.05)。低级别胶质瘤高危组早期替莫唑胺口服化疗药物,不良反应发生率低于高级别组(P<0.05)。结论低级别胶质瘤在IDH突变型、1p/19q缺失和VEGF阴性表达高于高级别组,低级别胶质瘤早期化疗不良反应低于高级别胶质瘤患者。 Objective To investigate the effects of IDH1,1P/19q,MGMT promoter methylation and VEGF on different grades of GLIOMAS.Methods 30 patients with Astrocytoma and oligodendrocyte tumors and Glioblastoma multiforme were selected,including 15 patients of high grade and 15 patients of low grade(Group A,Group B).The general condition of the patients,the clinical symptoms and signs of tumor-related functional loss,the imaging features before and after operation,the methylation of IDH1,1P/19q,MGMT promoter and VEGF were recorded The low-grade group was stratified according to the molecular pathological results and high-risk factors.The high-risk Group(Group C)and the high-grade Group(Group B)were treated with temozolomide(75 mg/kg/m^2,5 days)orally,the related adverse drug reactions were recorded.The statistical analysis of the data was carried out by using SPSS statistical software.P<0.05 showed that the difference was statistically significant.Results The KPS scores in Group A were higher than those in group B(P<0.05).The ratio of frontal lobe to temporal lobe was higher than that of frontotemporal lobe,parietal lobe and cerebellum(P<0.05),and the ratio of frontotemporal lobe to cerebellum was higher than that of parietal lobe and cerebellum(P<0.05).The patients with preoperative functional loss were significantly higher than those without functional loss(P<0.01).Group B was lower than group a(P<0.05)in patients≤40 years of age,IDH mutation and negative for VEGF.The incidence of adverse reactions of temozolomide in high risk group of low grade Glioma was lower than that in high grade group(P<0.05).Conclusion The Idh Mutation,1P/19q deletion and VEGF negative expression in low grade GLIOMAS were higher than those in high grade gliomas,and the early chemotherapy adverse reaction of low grade GLIOMAS was lower than that in high grade gliomas.
