注射用头孢哌酮他唑巴坦钠联合桑夏苓术汤治疗卒中相关性肺炎的疗效观察

Effect observation of injection cefoperazone tazobactam sodium combined with Sangxialingzhu Decoction in treatment for stroke associated pneumonia

目的探讨注射用头孢哌酮他唑巴坦钠联合桑夏苓术汤治疗卒中相关性肺炎的疗效。方法选取华北理工大学附属医院2017年9月~2019年2月卒中相关性肺炎患者122例,采用随机数字表法分为两组,对照组61例采用注射用头孢哌酮他唑巴坦钠治疗,观察组61例采用注射用头孢哌酮他唑巴坦钠联合桑夏苓术汤治疗,于治疗前后行生化指标检测、功能评分,比较两组病症改善时间、临床疗效、不良反应。结果两组治疗后C-反应蛋白、白细胞计数、降钙素原、白细胞介素-6、肿瘤坏死因子-α、临床肺部感染评分(CPIS)、神经功能缺损评分(NIHSS)、急性生理和慢性健康评分(APACHEⅡ)、改良Rankin评分较治疗前降低(P<0.05)。观察组治疗后C-反应蛋白、白细胞计数、降钙素原、白细胞介素-6、肿瘤坏死因子-α、CPIS评分、NIHSS评分、APACHEⅡ评分、改良Rankin评分均低于对照组(P<0.05)。观察组咳嗽消失时间、体温复常时间、肺部湿啰音消失时间、胸部影像复常时间、白细胞计数复常时间均短于对照组(P<0.05)。观察组总有效率高于对照组(P<0.05)。结论注射用头孢哌酮他唑巴坦钠联合桑夏苓术汤治疗卒中相关性肺炎的疗效显著,可减轻炎症反应,加速康复进程。

Objective To explore effect of injection cefoperazone tazobactam sodium combined with Sangxialingzhu Decoction in treatment for stroke associated pneumonia.Methods 122 patients with stroke associated pneumonia were selected in Affiliated Hospital of North China University of Science and Technology from September 2017 to February 2019,who were divided into two groups by random number table method.61 patients treated injection cefoperazone tazobactam sodium as control group.61 patients treated injection cefoperazone tazobactam sodium combined with Sangxialingzhu Decoction as observation group.Biochemical indexes and functional scores were measured before and after treatment.Disease improvement time,clinical effect,adverse reactions were compared between two groups.Results C-reactive protein,white blood cell count,procalcitonin,interleukin-6,tumor necrosis factorα,CPIS score,NIHSS score,APACHEⅡscore,modified Rankin score decreases in two groups after treatment(P<0.05).C-reactive protein,white blood cell count,procalcitonin,interleukin-6,tumor necrosis factorα,CPIS score,NIHSS score,APACHEⅡscore,modified Rankin score decreases in observation group were lower than those in control group after treatment(P<0.05).Cough disappearance time,body temperature recovery time,lung dampness rale disappearance time,chest image normalization time,white blood cell count recovery time in observation group were shorter than those in control group(P<0.05).Total effective rate in observation group was higher than that in control group(P<0.05).Conclusion Injection cefoperazone tazobactam sodium combined with Sangxialingzhu Decoction have significant effect in treatment for stroke associated pneumonia,which can reduce inflammatory reaction and accelerate rehabilitation process.
