进展性脑挫裂伤相关危险因素研究

Study on the Risk Factors of Progressive Brain Contusion

目的研究脑挫裂伤患者早期发生进展性脑挫裂伤的高危因素。方法回顾性收集脑挫裂伤患者149例,按脑挫裂伤病情是否发生进展分为非进展组80例和进展组69例,对相关危险因素进行单因素分析和多因素Logistic分析。结果单因素分析结果提示性别、入院时血糖、入院时格拉斯哥昏迷评分(GCS评分)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、活化部分凝血活酶时间(APTT)、系统性免疫炎症指数、中性粒细胞/淋巴细胞比值、血清钠、血钾水平是与脑挫裂伤进展密切相关。多因素Logistic回归分析提示入院GCS评分、系统性免疫炎症指数、血清钾水平是进展性脑挫裂伤的独立危险因素。结论入院GCS评分、系统性免疫炎症指数、血清钾水平是进展性脑挫裂伤的独立危险因素,应注意早期复查头颅CT,及时干预治疗。

Objective To study the high-risk factors of early progressive brain contusion in patients with brain contusion and laceration.Method 149 patients with brain contusion and laceration were retrospectively analyzed.Whether brain contusion and laceration occurred or not,they were divided into non-progressive group(80 cases)and progressive group(69 cases).And the related risk factors were analyzed by univariate and multivariate logistic analysis.Results Univariate analysis results suggested that gender,blood glucose at admission,Glasgow coma score(GCS score)at admission,alanine aminotransferase(ALT),aspartate aminotransferase(AST),activated partial thromboplastin time(APTT),systemic immune inflammation index,neutrophil/lymphocyte ratio,serum sodium,and potassium were closely related to the progression of brain contusion.Multivariate logistic regression analysis indicated that admission GCS score,systemic immune inflammation index,and serum potassium level were independent risk factors for progressive brain contusion.Conclusion Admission GCS score,systemic immune inflammation index,and serum potassium level are independent risk factors for progressive cerebral contusion and laceration.Attention should be paid to early reexamination of head CT and timely intervention and treatment.
