摘要
目的:探讨术中磁共振成像在神经外科的精准应用。随着科学的发展,多学科的交叉,在医学领域,对人类疾病的认识更加精准,磁共振的出现,是医学界的一大奇迹,进而在神经外科手术的需求中,大胆设想,诞生了术中MR,极大地解决了外科医生在手术时的盲点,丰富了外科医生的手术助手。磁共振无放射性,软组织分辨率高,对于神经系统成像效果佳;并可提供横冠矢等任意平面的影像,在术前,利用磁共振多参数、多序列对病灶的评估,在术中,借助于导航工作站可以随时更新导航,指导手术的进行,术后复查对脑功能的影响程度及恢复情况。资料与方法:总结北京大学国际医院从2017年2月开业以来,使用磁共振复合手术室完成近600例手术,其中包括胶质瘤207台,脑血管畸形197台,垂体瘤97台,立体定向活检80台,随机选取用磁共振复合手术室20台胶质瘤手术和常规手术室手术胶质瘤20台对比切除率。结果:根据以上数据结果显示,使用术中磁共振手术切除率高于常规手术切除率,且P<0.05,有统计学意义。结论:术中磁共振在中枢神经系统手术应用广泛[3-5],具有重要意义。
Objective:We explore the accurate application of intraoperative magnetic resonance imaging in neurosurgery.With the development of science and the crossing of multiple disciplines,people have a more accurate understanding of human diseases in the medical field.The appearance of magnetic resonance imaging is a great miracle discovery in the medical field.Hence,in the demand of neurosurgery,the bold assumption was made and the intraoperative MR was born,which greatly solved the blind spots of surgeons during the operation and enriched the surgical assistants of surgeons.MRI is non-radioactive,with high soft tissue resolution,and has good imaging effects on the nervous system.Images in any plane such as transverse coronal vector can be provided.Preoperatively,multi-parameters and multi-sequences of magnetic resonance can be used to evaluate the lesions.During the operation,the navigation can be updated at any time with the help of the navigation workstation to guide the operation.The degree of impact and recovery of postoperative reexamination on brain function is also provided.Methods Since the opening of peking university international hospital in february 2017,nearly 600 cases have been completed using mri compound operating rooms,including 207 gliomas,197 cerebrovascular malformations,and 80 stereotactic biopsies for 97 pituitary tumors.twenty gliomas operated in mri compound operating rooms and 20 gliomas operated in conventional operating rooms were randomly selected to compare the resection rates.Results According to the above data,the resection rate of intraoperative magnetic resonance surgery was higher than that of conventional surgery,and there was a statistical significance(P<0.05).Conclusion During the operation,magnetic resonance imaging is widely used in central nervous system,and it is of great significance.
