摘要
目的观察焠刺治疗老年膝骨关节炎的临床疗效及对关节腔液炎性细胞因子及关节功能的影响。方法将206例老年膝骨关节炎患者随机均分为治疗组(103例,行焠刺治疗)、对照组(103例,采用常规针刺治疗)。对比两组中医症候评分、Lysholm膝关节评分(LKS评分)及各项关节腔液炎性细胞因子[白细胞介素1(IL-1)、白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)]在治疗前后的变化,比较两组总有效率。结果治疗组总有效率为94.2%,高于对照组的85.4%,差异具有统计学意义(P<0.05)。两组治疗后中医症候评分、LKS评分及IL-1、IL-6、TNF-α水平较治疗前发生明显变化(P<0.05)。治疗组治疗后中医症候评分及IL-1、IL-6、TNF-α水平低于对照组,而LKS评分高于对照组,差异具有统计学意义(P<0.05)。结论焠刺是一种治疗老年膝骨关节炎的有效方法,能有效缓解患者疼痛与肿胀症状,降低关节腔液中炎性细胞因子水平。
Objective To investigate the clinical efficacy of Red-hot needling acupuncture for senile knee osteoarthritis and its effect on inflammatory cytokines in synovial fluid and joint function.Method Two hundred and six patients with senile knee osteoarthritis were randomized to a treatment group(103 cases,treated with Red-hot needling acupuncture)and a control group(103 cases,treated with conventional acupuncture).The TCM symptom scores,the Lysholm Knee Scale(LKS)scores and the inflammatory cytokine(interleukin-1(IL-1),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α))contents of synovial fluid were compared between the two groups before and after treatment.The total efficacy rates were compared between the two groups.Result The total efficacy rate was 94.2%in the treatment group,which was higher than 85.4%in the control group with a statistically significant difference(P<0.05).The TCM symptom score,the LKS score,and IL-1,IL-6 and TNF-αlevels changed significantly in the two groups after treatment compared with before(P<0.05).After treatment,the TCM symptom score,and IL-1,IL-6 and TNF-αlevels were lower and the L score higher in the treatment group than in the control group with statistically significant differences(P<0.05).Conclusion Red-hot needling acupuncture is an effective way to treat senile knee osteoarthritis.It can effectively relieve pain and swelling symptoms in the patients and reduce the levels of inflammatory cytokines in synovial fluid.
