3Hz重复经颅磁刺激联合舌压抗阻反馈训练对老年脑卒中患者吞咽功能及舌骨活动度的影响

Effect of 3Hz repetitive transcranial magnetic stimulation combined with tongue pressure resistance feedback training on swallowing function and hyoid activity of elderly patients after stroke

目的研究3Hz重复经颅磁刺激联合舌压抗阻反馈训练对老年脑卒中患者吞咽功能及舌骨活动度的影响。方法选取我院收治的126例老年脑卒中后吞咽障碍患者作为研究对象,随机分为对照组和观察组,每组63例。对照组给予常规摄食训练和舌压抗阻反馈训练,观察组给予3Hz重复经颅磁刺激联合舌压抗阻反馈训练,4周后对比两组患者临床疗效、吞咽功能和舌骨活动度。结果观察组在训练4周后有效率高于对照组(P<0.05);训练后观察组VFSS和FOIS升高、PAS降低幅度均高于对照组(P<0.05);训练后,观察组舌骨活动度提高幅度高于对照组(P<0.05)。结论3Hz重复经颅磁刺激联合舌压抗阻反馈训练对脑卒中后吞咽障碍患者效果确切,能够通过反复刺激促进神经通路的重建,调节吞咽中枢,提高脑卒中预后。

Objective To explore the effect of 3Hz repetitive transcranial magnetic stimulation combined with tongue pressure resistance feedback training on swallowing function and hyoid activity of elderly patients after stroke.Methods 126 elderly patients with dysphagia after stroke were selected and randomly divided into the control group and the observation group,63 cases in each group.The control group was given routine feeding training and tongue pressure resistance feedback training,while the observation group was given 3Hz repetitive transcranial magnetic stimulation com⁃bined with tongue pressure resistance feedback training.After 4 weeks,the clinical efficacy,swallowing function and hy⁃oid activity of the two groups were compared.Results After 4 weeks of training,the effective rate of the observation group was higher than that of the control group(P<0.05).After training,the increase of VFSS and FOIS and the decrease of PAS in the observation group were higher than those in the control group(P<0.05).After training,the activity of hyoid bone in the observation group was higher than that in the control group(P<0.05).Conclusion 3Hz repetitive transcrani⁃al magnetic stimulation combined with tongue pressure resistance feedback training is effective in the treatment of pa⁃tients with dysphagia after stroke.It can promote the reconstruction of nerve pathways through repeated stimulation,regu⁃late the swallowing center and improve the prognosis of stroke.
