骨科护士家庭功能与心理韧性在职业倦怠与抑郁症状关系中的多重中介作用

Multiple mediating effect of family function and resilience in relationship between job burnout and depressive symptoms in orthopedic nurses

目的探讨骨科护士职业倦怠、家庭功能、心理韧性和抑郁症状的现状及其关系。方法2019年3—12月,采用便利抽样法,抽取济南市5所三级甲等综合性医院136名骨科护士作为调查对象,采用一般资料问卷、职业倦怠问卷、简易心理韧性量表、家庭关怀指数量表和患者健康问卷抑郁量表进行调查,并对数据进行分析。结果骨科护士职业倦怠得分为63.65±4.23,家庭功能得分为7.46±2.35,心理韧性得分为26.29±5.99,抑郁症状得分为6.79±4.46;其职业倦怠与家庭功能、心理韧性均呈显著负相关(r=-0.213、-0.404,均P<0.01),职业倦怠与抑郁症状呈显著正相关(r=0.478,P<0.01),家庭功能、心理韧性与抑郁症状均呈显著负相关(r=-0.489、-0.610,均P<0.01),家庭功能与心理韧性呈显著正相关(r=0.597,P<0.01);心理韧性与家庭功能呈显著正相关(r=0.284,P<0.01)。家庭功能在职业倦怠与抑郁症状之间的中介效应值为0.041,占总效应的8.6%;心理韧性在职业倦怠与抑郁症状之间的中介效应值为0.101,占总效应的21.1%;家庭功能与心理韧性在职业倦怠与抑郁症状之间的链式中介效应值为0.040,占总效应的8.4%。结论骨科护士职业倦怠可以分别通过家庭功能、心理韧性的中介作用、家庭功能和心理韧性的链式中介作用间接影响抑郁症状。

Objective To investigate the status of job burnout,family function,resilience and depressive symptoms of orthopedic nurses,explore their relationship.Methods From march to December 2019,136 orthopedic nurses in five grade-A tertiary hospitals in Jinan City were selected by convenient sampling method as the survey objects.The general information questionnaire,Maslach burnout inventory,simple resilience scale,family care index scale and patient health questionnaire depression scale were used for the survey.And the data were analyzed.Results The scores of job burnout,family function,resilience and depressive symptoms in orthopedic nurses were 63.65±4.23,7.46±2.35,26.29±5.99 and 6.79±4.46 respectively.Their job burnout was significantly negatively correlated with the family function and resilience(r=-0.21,-0.404,both P<0.01),the job burnout was significantly positively correlated with the depression symptoms(r=0.478,P<0.01),the family function and resilience were significantly negatively correlated with the depression symptoms(r=-0.489,-0.610,both P<0.01),and the family function was significantly positively correlated with the resilience(r=0.597,P<0.01).There was a significant positive correlation between resilience and family function(r=0.284,P<0.01).The mediating effect value of family function between job burnout and depressive symptoms was 0.041,accounting for 8.6%of the total effect.The mediating effect value of resilience between job burnout and depressive symptoms was 0.101,accounting for 21.1%of the total effect.The chain mediating effect of family function and resilience between job burnout and depressive symptoms was 0.040,accounting for 8.4%of the total effect.Conclusion Job burnout of orthopedic nurses can indirectly affect depressive symptoms through the mediating effect of family function,resilience and the chain mediating effect of family function and resilience.
