大肠埃希菌及肺炎克雷伯菌耐药率与抗菌药物使用强度相关性分析

Correlation Between the Drug Resistance Rate of Escherichia Coli and Klebsiella Pneumoniae and the Antibiotics Use Density

目的探讨大肠埃希菌及肺炎克雷伯菌耐药性与抗菌药物使用强度的相关性,为临床合理使用抗菌药物提供参考.方法回顾性调查宜宾市第一人民医院2014年至2019年临床分离的大肠埃希菌及肺炎克雷伯菌对常用抗菌药物的耐药率,以及同期抗菌药物使用强度(AUD),探讨两者的相关性。结果该院大肠埃希菌及肺炎克雷伯菌分离率呈上升趋势,对碳青霉烯类抗菌药物的耐药率也有类似的增长趋势;大肠埃希菌耐药率与头孢曲松、头孢他啶、环丙沙星、左氧氟沙星、亚胺培南等的AUD呈正相关(P<0.05);肺炎克雷伯菌耐药率与头孢曲松、头孢他啶、亚胺培南等的AUD呈正相关(P<0.05)。结论―大肠埃希菌及肺炎克雷伯菌耐药率与抗菌药物AUD存在宏观量化关系,应加强抗菌药物监管,提高合理使用水平,减少细菌耐药。

Objective To investigalte the correlation between the drug resistance rate of Escherichia coli and Klebsiella mneumoniae andthe antibiotics use density(AUD),and to provide a hasis for the rational use of antibiotics in the clinical practice.Methods The AUDand drug resistance rale of Escherichia cali and Klebsiella pneumoniae isolated in The Fist People's Hospital of Yibin from 2014 to2019,as well as their correlation were retospectively investigated.Results The isolation rate of Escherihia wli and Klebsiella pmeumoniaein the hospital showed an upwand trend,and their mesistance rate to carbapenem antilacterial drugs also showed a similar growthtrend.The dnug resistance ate of Escherichia coli was positively corelated with the AUD of ceftiaxvone,cetazidime,cipofloxacin,lev-ofloxacin and imipenem(P<0.05).The drug resistance rale of Klebsiella pneumonine was positively corelated wih the AUD ofceftiaxone,ceftazilime and imipenem(P<0.05).Conclusion There is a macro-quanitative relationship between the dng resis-tance ate of Escherihia coli and Klebsiella pmeumonine and the AUD.The supervision of antimicrobial drugs should be strengthened toimprove the level of rational use of antimicrobial drugs and reduce the occuence of bacterial resistance.
