结直肠癌患者血清HPSE及VEGF表达水平检测及临床意义研究

The expression and clinical significance of HPSE and VEGF in serum of patients with colorectal cancer

目的研究乙酰肝素酶(HPSE)及血管内皮生长因子(VEGF)在结直肠癌患者血清中的表达情况,并分析其与结直肠癌临床病理关系。方法选取2016年4月至2019年2月在该院经手术及病理证实为结直肠癌患者118例作为研究组,选取健康志愿者60例作为对照组。采用酶联免疫吸附试验(ELISA)检测2组血清标本中HPSE及VEGF表达水平。结果研究组血清HPSE及VEGF的表达水平显著高于对照组,差异均有统计学意义(P<0.05);研究组血清HPSE及VEGF高表达与肿瘤分化程度、肿瘤分期、淋巴结转移密切相关,与患者性别、年龄及肿瘤部位无关;肿瘤大小与VEGF表达相关而与HPSE表达无关。血清HPSE与VEGF指标联合检测诊断结直肠癌的受试者工作特征曲线(ROC曲线)值为0.86,高于单独检测血清HPSE的0.76与单独检测血清VEGF的0.80。血清HPSE与VEGF的表达水平呈显著正相关。结论HPSE及VEGF联合检测能够提高结直肠癌诊断水平,可成为结直肠癌患者筛查、判断预后及指导治疗的指标。

Objective To study the expression of heparanase(HPSE)and vascular endothelial growth factor(VEGF)in the serum of patients with colorectal cancer,and analyze their clinicopathological relationship with colorectal cancer.Methods A total of 118 patients with colorectal cancer confirmed by surgery and pathology in the hospital from April 2016 to February 2019 were selected as the study group,and 60 healthy volunteers were selected as the control group.Enzyme-linked immunosorbent assay(ELISA)was used to detect the expression levels of HPSE and VEGF in serum samples of the two groups.Results The expression levels of serum HPSE and VEGF in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The high expression of serum HPSE and VEGF in the study group was closely related to the degree of tumor differentiation,tumor stage,and lymph node metastasis,but not to the patient′s gender,age,and tumor location.Tumor size was related to VEGF expression but not to HPSE expression.The receiver operating characteristic curve(ROC curve)value of the combined detection of serum HPSE and VEGF indicators for the diagnosis of colorectal cancer was 0.86,which were higher than the 0.76 of serum HPSE and 0.80 of serum VEGF.There was a significant positive correlation between the expression levels of serum HPSE and VEGF.Conclusion The combined detection of the two indicators can improve the diagnosis of colorectal cancer,and can be used as an indicator for screening patients with colorectal cancer,judging the prognosis and guiding treatment.
