基于不同量表探讨慢性肝炎患者生命质量的内涵及健康改善研究

Discussion on the Connotation of Quality of Life and Research on Health Improvement in Patients with Chronic Hepatitis Based on Different Scales

目的分析慢性肝炎患者的生命质量状况并探讨不同量表在生命质量测量中的内涵差异及健康改善途径。方法分别采用EQ-5D-5L、ICECAP-A和EBQoL量表测量240例慢性肝炎患者的生命质量,同时采用因子分析、Bland-Altman分析和Spearman相关分析探讨测量结果的内涵、一致性及相关性。结果 EQ-5D-5L、ICECAP-A和EBQoL量表测量的受访者生命质量总体得分分别为(89.15±13.84)分、(71.10±18.32)分和(65.17±17.93)分,各测量结果间有一定的正相关性(r=0.173~0.435,P<0.01),且一致性较高。三类量表的测量结果主要加载于4个公因子,分别主要反映影响生命质量的生理健康、与个人生活环境和条件密切相关的心理健康、与社会环境相关的心理健康以及自然环境因素。结论慢性肝炎患者总体的生命质量水平不高,对其进行健康改善除了应关注个人生理及心理健康外,还需要同时关注患者所处的宏观外界社会环境和生存条件。

OBJECTIVE To analyze the quality of life(QoL) in patients with chronic hepatitis and to explore differences of connotation and ways of health improvement between measuring different QoL scales.METHODS EQ-5 D-5 L,ICECAP-A,EBQoL scales were used to measure the QoL in patients with chronic hepatitis(n=240).Meanwhile,factor analysis,Bland-Altman analysis and Spearman analysis were used to explore the connotation,consistency and correlation of measuring results.RESULTS The overall QoL scores of the respondents measured by EQ-5 D-5 L,ICECAP-A and EBQoL scales were(89.15±13.84),(71.10±18.32),(65.17±17.93),respectively.There were positive correlation(r=0.173~0.435.P<0.01) and higher consistency between the measuring results.The result of scales was mainly loaded into 4 common factors,which affect the physiological health of the QoL,the mental health that was closely related to the personal living and social environment and the natural environment factors.CONCLUSION The overall QoL scores of patients with chronic hepatitis was not high.To improve health of patients with chronic hepatitis,it not only needed to focus on personal physical and mental health,but also pay attention to the external social environment and living condition of patients.
