附子理中汤加减联合针灸治疗糖尿病胃轻瘫20例临床观察

Clinical Observation on 20 Cases of Diabetic Gastroparesis Treated with The Jia Jian Fuzi Lizhong Decoction Combined with Acupuncture

目的:观察加减附子理中汤联合针灸治疗脾胃阳虚型糖尿病胃轻瘫(DGP)的临床疗效及对胃泌素-17(G-17)的影响。方法:将40例脾胃阳虚型DGP患者随机分为对照组和观察组各20例,对照组予甲钴胺和枸橼酸莫沙必利口服治疗,观察组予加减附子理中汤联合针灸治疗,两组均以4周为疗程。观察两组治疗前后中医证候评分、4 h胃排空率、血糖及胃泌素-17水平,并比较两组临床总有效率及随访1月后复发率。结果:治疗后,观察组4 h胃排空率显著优于对照组(P<0.01);中医证候积分、胃泌素-17、血糖水平、临床总有效率及随访1月后复发率均优于对照组(P<0.05)。结论:加减附子理中汤联合针灸治疗脾胃阳虚型DGP可明显改善临床症状,下调胃泌素水平,加速胃排空进程,利于血糖控制,且复发率低,疗效确切。

Objective To observe the clinical effects of The Jia Jian Fu Zi Li Zhong decoction combined with acupuncture in the treatment of DGPwith deficient yang of spleen and stomach and its effect on gastrin-17(G-17).Methods The40 patients with diabetic gastroparesis due to deficient yang of spleen and stomach were randomly divided into Observation group and control group(N=20).The control group was treated with Mecobalamin and Mosapride citrate orally for 4 weeks.The Observation group was treated with The Jia Jian Fu Zi Li Zhong decoction combined with acupuncture.The TCM syndrome scores,gastric emptying rate and serum gastrin level were observed before and after treatment,To compare the clinical total effective rate and the recurrence rate after one month following-up.Results After treatment,the gastric emptying rate of the Observation group was significantly higher than that of the control group(P<0.01),and the TCM Syndrome Score,gastrin-17,blood glucose level,the clinical total effective rate and the recurrence rate of the treatment group were significantly higher than those of the control group(P<0.05).Conclusion The Jia Jian Fu Zi Li Zhong decoction combined with acupuncture in the treatment of GDP with deficient yang of spleen and stomach can significantly improve the clinical symptoms,reduce gaprotin level,accelerate gastric emptying process,benefit blood sugar control,and the recurrence rate is low,with definite curative effect.
