甘精胰岛素与中效人胰岛素治疗老年2型糖尿病的效果评价

Effect evaluation of insulin glargine and moderate effective human insulin in the treatment of elderly type 2 diabetes mellitus

目的比较甘精胰岛素与中效人胰岛素治疗老年2型糖尿病的效果。方法选取2017年1月至2019年1月本院收治的400例老年2型糖尿病患者作为研究对象,依据随机对照原则分为两组,每组200例。对照组给予速效胰岛素+中效人胰岛素治疗,观察组给予速效胰岛素+甘精胰岛素治疗。比较两组患者入院时及治疗1周后血糖代谢指标变化情况、血糖达标时间、胰岛素日用量以及用药期间低血糖发生率。结果治疗1周后,观察组患者空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)水平均显著低于对照组,差异有统计学意义(P<0.05);观察组患者血糖达标时间短于对照组,胰岛素日用量低于对照组,差异有统计学意义(P<0.05);观察组用药期间低血糖发生率低于对照组,差异有统计学意义(P<0.05)。结论甘精胰岛素配合速效胰岛素对老年2型糖尿病血糖控制效果更佳,低血糖发生率也更低,值得临床推荐。

Objective To compare the effect of insulin glargine and moderate effective human insulin in the treatment of elderly type 2 diabetes mellitus.Methods 400 elderly patients with type 2 diabetes admitted to our hospital from January 2017 to January 2019 were selected as the research objects,and they were divided into two groups according to the principle of random control,with 200 cases in each group.The control group was treated with quick acting insulin and moderate effective human insulin,while the observation group was treated with quick acting insulin and insulin glargine.The changes of blood glucose metabolism indexes,the time of reaching blood glucose standard,the daily dosage of insulin and the incidence of hypoglycemia were compared between the two groups at admission and 1 week after treatment.Results After 1 week of treatment,the levels of FPG,2 h PG and HbA1c in the observation group were significantly lower than those in the control group(P<0.05);the time of blood glucose reaching the standard in the observation group was shorter than that in the control group,and the daily dosage of insulin was lower than that in the control group(P<0.05);the incidence of hypoglycemia in the observation group was lower than that in the control group(P<0.05).Conclusion Insulin glargine combined with quick acting insulin has better blood glucose control effect and lower incidence of hypoglycemia in elderly patients with type 2 diabetes,which is worthy of clinical recommendation.
