我院胃食管反流病影响因素调查及胃镜表现

Investigation on influencing factors of gastroesophageal reflux disease in Jiangxi area and gastroscopic manifestations

目的探讨胃食管反流病(GERD)影响因素及胃镜表现。方法选择2019年1月至12月经我院消化内科诊断为GERD患者500例为GERD组,选择同期就诊于我院的非消化道疾病的患者500例为对照组。分析GERD影响因素,观察胃镜下GERD患者胆汁反流及食管炎表现。结果Logistic回归分析:年龄≥60岁、体重指数≥24kg/m2、喜油腻食物、喜食辛辣、吸烟、幽门杆菌(Hp)感染是GERD发生的独立危险因素(P<0.05且OR≥1);经胃镜检查显示,胆汁反流程度分级:Ⅰ度178例(35.60%),Ⅱ度126例(25.20%),Ⅲ度196例(39.20%);食管炎分级:A级219例(43.80%),B级176例(35.20%),C级61例(12.20%),D级44例(8.80%)。结论GERD发生与多种因素密切相关且胆汁反流程度、食管炎分级高,临床需针对各危险因素制定相应的防治措施,以降低GERD发生率和并发症发生率,改善患者预后。

Objective To explore the influencing factors and gastroscopic manifestations of gastroesophageal reflux disease(GERD)in Jiangxi area.Methods 500 patients diagnosed with GERD by the Department of Gastroenterology of our hospital from January to December 2019 were selected as the GERD group,and a total of 500 patients with non-gastrointestinal diseases who visited our hospital at the same time were selected as the control group.Analyze the influencing factors of GERD in Jiangxi area and observe the manifestations of bile reflux and esophagitis in GERD patients under gastroscopy.Results Logistic regression analysis:age≥60 years old,body mass index≥24kg/m2,preference for greasy food,preference for spicy food,smoking,and Helicobacter pylori(Hp)infection were independent risk factors for GERD in Jiangxi(P<0.05 and OR≥1 Gastroscopy showed that the degree of bile reflux was graded:178 cases(35.60%)of degree I,126 cases(25.20%)of degree II,196 cases(39.20%)of degree III;esophagitis grade:A grade of 219 cases(43.80)%),176 cases of grade B(35.20%),61 cases of grade C(12.20%),and 44 cases of grade D(8.80%).Conclusion The occurrence of GERD in Jiangxi area is closely related to many factors,and the degree of bile reflux and esophagitis are high.It is necessary to develop corresponding prevention and treatment measures for each risk factor in order to reduce the incidence of GERD and the incidence of complications,and improve the prognosis of patients.
