老年男性骨质疏松症患者1,25-(OH)2D3、胰岛素样因子、睾酮的表达及其与骨代谢指标的相关性 被引量:9

摘要 目的探讨老年男性骨质疏松症(osteoporosis,OP)患者25羟维生素D[1,25-(OH)2D3]、胰岛素样因子、睾酮的表达及其与骨代谢指标的相关性。方法选择2016年7月~2019年6月本院骨科收治的45例老年男性OP患者纳入观察组,另择同期45例健康老年男性作为对照组。比较两组患者1,25-(OH)2D3、胰岛素样因子、睾酮、骨形成标志物及骨吸收标志物水平,并分析其相关性。结果观察组的1,25-(OH)2D3、胰岛素样因子、睾酮、骨保护素(OPG)、骨碱性磷酸酶(BALP)、骨钙素(BGP)、I型前胶原氨基端原肽(PINP)水平均显著低于对照组(P<0.05);观察组患者核因子κB受体活化因子配体(RANKL)水平、抗酒石酸酸性磷酸酶-5b(TRACP-5b)水平、β-胶原降解产物(β-CYX)、基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)水平均高于对照组(P<0.05)。Pearson相关结果显示,1,25-(OH)2D3、胰岛素样因子、睾酮水平与血清骨形成标志物(OPG、BALP、BGP、PINP)水平呈正相关(P<0.05),与血清骨吸收标志物(RANKL、TRACP-5b、MMP-2、MMP-9、β-CYX)水平呈负相关(P<0.05)。结论老年男性OP患者1,25-(OH)2D3、胰岛素样因子、睾酮的水平较低,且其表达与骨质流失呈负相关。 Objective To explore the expressions of 25-hydroxyvitamin D3[1,25-(OH)2D3],insulin-like factor,testosterone and their correlation with bone metabolism in elderly male patients with osteoporosis(OP).Methods From July 2016 to June 2019,45 elderly male patients with OP were enrolled in the observation group,and 45 healthy elderly men were selected as the control group.The levels of 1,25-(OH)2D3,insulin-like factors,testosterone,bone formation markers and bone resorption markers in patients were compared,and their correlation was analyzed.Results The levels of 1,25-(OH)2D3,insulin-like factors,testosterone,osteoprotegerin(OPG),bone alkaline phosphatase(BALP),bone gla protein(BGP)and I-type precollagen amino-terminal propeptide(PINP)in the observation group were significantly lower than those in the control group(P<0.05).The levels of activation factor ligand(RANKL),tartrate-resistant acid phosphatase-5b(TRACP-5b),β-carboxy-terminal peptide(β-CYX),matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase-9(MMP-9)in the observation group were higher than those in the control group(P<0.05).The results of Pearson correlation showed that the levels of 1,25-(OH)2D3,insulin-like factor and testosterone were positively correlated with the levels of serum bone formation markers(OPG,BALP,BGP,PINP)(P<0.05),and negatively correlated with the levels of serum bone resorption markers(RANKL,TRACP-5b,MMP-2,MMP-9,β-CYX,P<0.05).Conclusion The levels of 1,25-(OH)2D3,insulin-like factor and testosterone in elderly male patients with osteoporosis are low,and their expressions are negatively correlated with bone loss.
