化脓灸辅助治疗对乳腺癌术后患者免疫功能的影响

Effect of adjuvant suppurative moxibustion therapy on immune function in postoperative patients with breast cancer

目的探讨化脓灸辅助治疗对乳腺癌术后患者免疫功能的影响。方法将146例乳腺癌术后患者随机分为6壮灸治疗组52例、9壮灸治疗组51例和不灸治疗组43例,6壮灸治疗组与9壮灸治疗组进行常规化疗的同时给予相应剂量的化脓灸治疗,不灸治疗组仅进行常规化疗,疗程均为7 d。观察化脓灸的直接治疗效果,并在治疗前和治疗7 d后采集患者外周血检测T淋巴细胞亚群、树突状细胞(DC)和自然杀伤细胞(NK)Ⅲ型以及相关细胞因子水平。结果治疗后,6壮灸治疗组和9壮灸治疗组起泡流脓率均大于90%,但两组差异无统计学意义(P>0.05);9壮灸治疗组患者的疤痕大于6壮灸治疗组(P<0.05)。与治疗前相比,6壮灸治疗组和9壮灸治疗组治疗后外周血CD3^+、CD4^+、CD11c^+CD123^+、CD16^+56^+细胞百分率、CD4^+/CD8^+比值、白细胞介素(IL)-12水平均升高,血清转化生长因子β1(TGF-β1)水平降低,但仅6壮灸治疗组IL-10水平降低(均P<0.05)。结论化脓灸辅助治疗能够明显改善乳腺癌患者外周血T淋巴细胞亚群、DC和NK细胞数量,同时上调患者血清IL-12水平,下调血清IL-10和TGF-β1水平,6壮灸治疗剂量的临床安全性较好。

Objective To explore the effect of adjuvant suppurative moxibustion therapy on the immune function of postoperative breast cancer patients.Methods A total of 146 postoperative breast cancer patients were randomly divided into 6 moxa-cone treatment group(n=52),9 moxa-cone treatment group(n=51)and moxibustion-naive treatment group(n=43).The 6 and 9 moxa-cone treatment groups were treated with suppurative moxibustion with corresponding doses and conventional chemotherapy simultaneously,while the moxibustion-naive treatment group only received conventional chemotherapy,and the treatment lasted for seven days in the three groups.The direct therapeutic effect of suppurative moxibustion was observed,and the levels of T lymphocyte subsets,dendritic cell(DC)and type Ⅲ natural killer cell(NK)as well as the levels of related cytokines were detected in peripheral blood collected from the patients before treatment and seven days after treatment.Results After treatment,the 6 and 9 moxa-cone treatment groups exhibited a blistering and abscess rate higher than 90%,but no statistically significant difference was found in the rate between the two groups(P>0.05);the 9 moxa-cone treatment group had greater size of scar than the 6 moxa-cone treatment group(P<0.05).Compared with pre-treatment,the 6 and 9 moxa-cone treatment groups had elevated percentages of CD3^+,CD4^+,CD11c^+CD123^+ and CD16^+56^+ cells in peripheral blood,increased CD4^+/CD8^+ ratio,higher interleukin(IL)-12 level,and reduced level of serum transforming growth factorβ1(TGF-β1)after treatment,however,only IL-10 level was decreased in the 6 moxa-cone treatment group(all P<0.05).Conclusion The adjuvant therapy with suppurative moxibustion can significantly improve the numbers of T lymphocyte subsets,DC and NK cells in peripheral blood of breast cancer patients,simultaneously,up-regulate serum IL-12 level and down-regulate serum IL-10 and TGF-β1 levels in the patients.The therapy of 6 moxa-cone moxibustion has a favorable clinical safety.
