空气波压力循环治疗仪辅助低分子肝素治疗人工全膝关节置换术患者临床观察 被引量:1

Clinical observation of air wave pressure circulation therapy apparatus assisted low molecular heparin in treating patients undergoing artificial total knee arthroplasty

摘要 目的人工全膝关节置换术是临床常见骨科手术方式,但术后患者容易出现下肢深静脉血栓,影响患者术后恢复。本研究分析空气波压力循环治疗仪辅助低分子肝素治疗人工全膝关节置换术后患者临床效果。方法选取本院2018-01-01-2019-02-28收治的62例行人工全膝关节置换术患者为研究对象,按照组间年龄、性别和临床类型等因素均衡的原则分为2组,各31例。对照组予以低分子肝素预防下肢深静脉血栓,研究组采用空气波压力循环治疗仪辅助低分子肝素治疗。比较两组临床疗效、患侧和健侧肢周径差、纤维蛋白原(fibrinogen,FIB)、活化部分凝血酶原时间(activated partial thromboplastin time,APTT)、D-二聚体以及疼痛评分。结果研究组患者临床总有效率为93.55%,高于对照组的74.19%,差异有统计学意义,χ2=4.292,P=0.038。干预后,研究组患侧和健侧肢周径差为(2.21±0.26)cm,低于对照组的(3.42±0.17)cm,t=4.459,P<0.001。干预后,研究组FIB为(2.74±0.19)g/L,低于对照组的(2.82±0.13)g/L,t=1.900,P=0.031;APTT为(25.41±5.58)s,低于对照组的(28.13±5.69)s,t=1.935,P=0.029;D-二聚体为(0.42±0.13)μg/mL,低于对照组的(0.54±0.21)μg/mL,t=2.705,P=0.004。结论空气波压力循环治疗仪辅助低分子肝素可有效改善人工全膝关节置换术患者下肢血液循环,提高临床疗效。 OBJECTIVE Artificial total knee arthroplasty is a common clinical orthopedic surgery,but patients are prone to deep vein thrombosis of the lower extremities after surgery,which affects the recovery of patients after surgery.This study analyzed the clinical effects of air wave pressure circulation therapy instrument assisted low molecular heparin in the treatment of patients after artificial total knee replacement.METHODS A total of 62patients undergoing artificial total knee arthroplasty who were admitted to this hospital from January 1,2018to February 28,2019were selected as the research objects.They were divided into 2groups according to the principle of balance between age,gender,and clinical type(each 31cases).The control group was given low-molecular-weight heparin to prevent deep venous thrombosis in the lower extremities,and the research group was treated with an air wave pressure cycle therapy device to assist low-molecular-weight heparin.The clinical efficacy,the difference in circumference of the affected and uninfected limbs,fibrinogen(FIB),activated partial thromboplastin time(APTT),D-dimer and annual pain scores were compared.RESULTS The total clinical effective rate of patients in the study group was 93.55%,which was higher than that of the control group(74.19%),and the difference was statistically significant,χ2=4.292,P=0.038.After intervention,the difference in circumference of the affected and uninfected limbs in the study group was(2.21±0.26)cm,which was lower than that of the control group(3.42±0.17)cm,t=4.459,P<0.001.After intervention,the FIB of the study group was(2.74±0.19)g/L,which was lower than that of the control group(2.82±0.13)g/L,t=1.900,P=0.031.APTT was(25.41±5.58)s,which was lower than that of the control group(28.13±5.69)s,t=1.935,P=0.029.D-dimer was(0.42±0.13)μg/ml,which was lower than that of the control group(0.54±0.21)μg/ml,t=2.705,P=0.004.CONCLUSION The air wave pressure circulation therapy instrument assisted by low molecular weight heparin can effectively improve the blood circulation of the lower limbs of patients undergoing artificial total knee arthroplasty,and improve the clinical efficacy.
