摘要
目的液基细胞学检测(liquid-based cytology test,LCT)广泛用于肿瘤细胞检测,细胞块免疫组化可丰富细胞学鉴别诊断信息。本研究分析LCT与细胞块免疫组化联合诊断肺癌临床价值。方法回顾性分析2017-03-06-2019-03-12于本院就诊的73例初期诊断为疑似肺癌患者的临床资料,患者均经LCT和细胞块免疫组化检查,并记录病理组织检查结果。以病理组织检查结果为“金标准”,评估LCT、细胞块免疫组化单项与联合检测诊断肺癌敏感性、特异性和准确度。结果LCT和免疫组化联合检测敏感性为98.46%,高于LCT(70.77%)和免疫组化单独检测(92.31%),χ2=20.506,P<0.001。LCT和免疫组化联合检测准确度为91.78%,高于LCT(72.60%)和免疫组化单独检测(90.41%),χ2=13.059,P=0.002。结论采用LCT联合细胞块免疫组化诊断肺癌的价值较为显著,可在一定程度上提高诊断敏感性和准确度。
OBJECTIVE Liquid-based cytology test(LCT)is widely used in tumor cell detection,and cell block immunohistochemistry can enrich cytological differential diagnosis information.This study analyzed the clinical value of LCT and cell block immunohistochemistry in the diagnosis of lung cancer.METHODS Clinical data of 73patients with suspected lung cancer who were diagnosed as suspected lung cancer in our hospital from March 6,2017to March 12,2019were retrospectively analyzed.All patients were examined by LCT and cell block immunohistochemistry,and the results of histopathological examination were recorded.Taking the results of histopathological examination as the"gold standard",the sensitivity,specificity and accuracy of LCT,cell block immunohistochemistry single and combined detection in the diagnosis of lung cancer were assessed.RESULTS The combined detection sensitivity of LCT and immunohistochemistry was 98.46%,which was higher than that of LCT(70.77%)and immunohistochemistry alone(92.31%),χ2=20.506,P<0.001.The combined detection accuracy of LCT and immunohistochemistry was 91.78%,which was higher than that of LCT(72.60%)and immunohistochemistry alone(90.41%),χ2=13.059,P=0.002.CONCLUSION The value of LCT combined with cell block immunohistochemistry in the diagnosis of lung cancer is more significant,which can improve the diagnostic sensitivity and accuracy to a certain extent.
