阿奇霉素联合异丙托溴铵对支原体肺炎患儿免疫功能及炎性因子的影响

Effects of azithromycin combined with ipratropium bromide on immune function and inflammatory factors in children with mycoplasma pneumonia

摘要 目的观察阿奇霉素联合异丙托溴铵对支原体肺炎患儿免疫功能及炎性因子的影响。方法选择2014年6月-2019年5月江西省大余县人民医院收治的支原体肺炎患儿60例,采用随机数字表法分为观察组(n=30)和对照组(n=30)。2组均行常规对症治疗,在此基础上对照组给予阿奇霉素治疗,观察组在对照组治疗的基础上联合异丙托溴铵治疗。比较2组患儿症状改善时间、治疗前后免疫功能指标及炎性因子水平。结果观察组肺部啰音消失时间、喘息缓解时间、咳嗽缓解时间及热退时间均短于对照组(P <0. 05或P <0. 01);治疗后,2组患儿血清免疫球蛋白A(Ig A)、Ig G、Ig M、C反应蛋白(CRP)及降钙素原(PCT)水平均低于治疗前,且观察组低于对照组,差异均有统计学意义(P <0. 05或P <0. 01)。结论阿奇霉素联合异丙托溴铵治疗支原体肺炎效果较好,有利于增强患儿免疫力,调节血清炎性因子水平,促进患儿恢复,值得临床推广应用。 Objective To observe the effect of azithromycin combined with ipratropium bromide on the immune function and inflammatory factors in children with mycoplasma pneumonia. Methods 60 children with mycoplasma pneumonia admitted to the People’s Hospital of Dayu County,Jiangxi Province from June 2014 to May 2019 were selected and divided into observation group( n = 30) and control group( n = 30). Both groups were treated with conventional symptomatic treatment. On this basis,the control group was given azithromycin treatment,and the observation group was treated with ipratropium bromide on the basis of the treatment of the control group. The time of symptom improvement,immune function indexes and inflammatory factor levels before and after treatment were compared between the two groups. Results The disappearance time of pulmonary rales,wheezing relief time,cough relief time,and fever subsidence time in the observation group were shorter than those in the control group,the differences were statistically significant( P < 0. 05 or P < 0. 01). After treatment,the serum immunoglobulin A( Ig A),Ig G,Ig M,C-reactive protein( CRP) and procalcitonin( PCT) levels of the two groups were lower than before treatment,and the observation group was lower than the control group,the difference were statistically significant( P < 0. 05 or P < 0. 01). Conclusion Azithromycin combined with ipratropium bromide has a good effect in the treatment of mycoplasma pneumonia,which is beneficial to enhance the immunity of children,regulate the level of serum inflammatory factors,and promote the recovery of children. It is worthy of clinical application.
