摘要
目的探讨白噪声在缓解早产儿视网膜病变玻璃体腔内注射雷珠单抗所致疼痛的效果。方法选取2019年1月至12月住院的早产儿64例作为研究对象,按照入院床位的单双号顺序分为干预组(n=34)和对照组(n=30)。对照组在表面麻醉后给予包被包裹,进行体位安抚,干预组在进行表面麻醉后,眼部消毒前3 min使用外音播放器循环播放白噪声,直至注射雷珠单抗3 min后结束播放。比较干预组与对照组患儿在注射雷珠单抗前3 min、注射后1 min、3 min的心率、经皮血氧饱和度变化和疼痛评分数值。结果玻璃体腔内注射雷珠单抗注射前3 min两组早产儿心率、血氧饱和度和疼痛评分差异无统计学意义(P>0.05);干预组使用表面麻醉结合白噪声干预下行玻璃体腔内注射雷珠单抗注射后1 min、3 min的心率低于对照组,经皮血氧饱和度高于对照组,早产儿疼痛评估量表得分均低于对照组(P<0.01)。结论表面麻醉结合白噪声进行干预能使早产儿玻璃体腔注射雷珠单抗的心率、经皮血氧饱和度更平稳,能缓解玻璃体腔注射时的操作性疼痛,在临床中有借鉴意义。
Objective To explore the effect of white noise on the pains caused by intravitreal injection of rapunzumab in preterm infants with retinopathy of prematurity.Methods A total of 64 preterm infants admitted from January to December 2019 were chosen as research subjects and randomly divided into:control group(n=30)and treatment group(n=34).Infants in the control group were swaddled and comfortably positioned after local anesthesia.Infants in the treatment group were treated with white noise continuously playing 3 min before disinfection until 3 min after the injection.Heart rate,SpO2 and pain score were compared 3 min prior-injection,1 and 3 min post-injection.Results There was no significant difference in pain score,heart rate and SpO2 between the two groups at 3 min before intravitreal injection(P>0.05).Heart rates 1 and 3 min post-injection and pain score in the treatment of infants were lower than those in control group.SpO2 in the treatment group at 1 and 3 min post-injection were higher than that in the control group(P<0.01).Conclusion White noises can stabilize heart rates and SpO2 and reduce acute pain of preterm infants treated by intravitreal injection of rapunzumab,which can be used clinically.
