摘要
目的研究水通道蛋白1 (AQP1)和水通道蛋白5 (AQP5)在外阴上皮内非瘤样病变和外阴鳞癌组织中表达情况并探讨其意义。方法选取2013年1月-2014年9月医院收治的原发外阴鳞癌患者84例为观察组,另取同期外阴上皮内非瘤样病变经病理检测证实无癌变患者84例为对照组。采用免疫组化法检测外阴上皮组织AQP1和AQP5蛋白表达情况,采用Kaplan-Meier生存分析AQP1和AQP5蛋白表达与外阴鳞癌患者5年生存率的关系,采用COX比例模型分析影响外阴鳞癌患者预后的危险因素。结果与对照组比较,观察组患者外阴上皮组织中AQP1和AQP5蛋白阳性表达率均显著升高,差异有统计学意义(P<0.05)。AQP1和AQP5蛋白表达与外阴鳞癌患者年龄、肿瘤位置、肿瘤直径无关(P>0.05),与分化程度、TNM分期、淋巴结转移有关(P<0.05)。AQP1蛋白阳性表达组外阴鳞癌患者5年总生存率55.56%显著低于阴性表达组患者的93.33%,AQP5蛋白阳性表达患者5年总生存率55.77%显著低于阴性表达患者的90.63%,差异均有统计学意义(P<0.05)。COX多因素分析发现,FIGO分期高、淋巴结转移、外阴上皮组织中AQP1及AQP5蛋白阳性表达是影响外阴鳞癌患者不良预后的独立危险因素(P<0.05)。结论 AQP1和AQP5蛋白在外阴鳞癌患者外阴上皮组织中阳性表达率高,且与分化程度、TNM分期、淋巴结转移及5年生存率有关,是影响外阴鳞癌患者预后的独立危险因素。
Objective To research the expressions of aquaporin 1 (AQP1) and aquaporin 5 (AQP5) in non-neoplastic lesions of vulvar epithelium and vulvar squamous cell carcinoma,explore the significance. Methods From January 2013 to September 2014,84 patients with primary vulvar squamous cell carcinoma treated in Maternal and Child Health Care Hospital of Anji County were selected as observation group,84 patients with non-neoplastic lesions of vulvar epithelium confirmed by pathological examination were selected as control group. The expressions of AQP1 and AQP5 in vulvar epithelium were detected by immunohistochemistry,Kaplan-Meier survival analysis was used to analyze the relationships between AQP1 and AQP5 proteins expressions and 5-year survival rate of vulvar squamous cell carcinoma patients,COX proportional model was used to analyze the risk factors affecting prognosis of patients with vulvar squamous cell carcinoma.Results Compared with control group,the positive expression rates of AQP1 and AQP5 proteins in vulvar epithelium of observation group increased significantly (P<0. 05). The expressions of AQP1 and AQP5 proteins were not correlated with age,location,and diameter of vulvar squamous cell carcinoma (P> 0. 05),but the expressions of AQP1 and AQP5 proteins were correlated with differentiation degree,TNM stage,and lymph node metastasis (P<0. 05). The 5-year overall survival rate of vulvar squamous cell carcinoma patients with positive expression of AQP1 protein was 55. 56%,which was statistically significantly lower than that of vulvar squamous cell carcinoma patients with negative expression of AQP1 protein (93. 33%);the 5-year overall survival rate of vulvar squamous cell carcinoma patients with positive expression of AQP5 protein was 55. 77%,which was statistically significantly lower than that of vulvar squamous cell carcinoma patients with negative expression of AQP5 protein (90. 63%) (P<0. 05). COX multivariate analysis showed that high FIGO stage,lymph node metastasis,positive expressions of AQP1 and AQP5 proteins in vulvar epithelium were independent risk factors of adverse prognosis of vulvar squamous cell carcinoma patients (P< 0. 05). Conclusion AQP1 and AQP5 are highly expressed in vulvar epithelial tissues of vulvar squamous cell carcinoma patients,which are related to differentiation degree,TNM stage,lymph node metastasis,and 5-year survival rate,which are independent risk factors affecting prognosis of vulvar squamous cell carcinoma patients.
