男同/双性恋者抑郁状况及儿童期虐待对其影响的研究

Status of depression and its relationship with childhood trauma among men having sex with men with homosexual and bisexual behavior

目的了解广州市男同/双性恋者的抑郁状况,探讨儿童期虐待对其抑郁症状的影响情况,为差异化干预提供线索。方法采用方便抽样,对到广州市越秀区疾病预防控制中心和岭南伙伴社区支持中心接受免费艾滋病自愿咨询检测服务的男男性行为者(MSM)进行匿名式问卷调查。结果共调查424名MSM,抑郁症状检出率为43.87%(186人)。其中男同性恋者占70.52%(299人),男双性恋者占29.48%(125人),两者的抑郁症状检出率分别为41.81%(125人)和48.80%(61人),但差异无统计学意义。男同性恋者儿童期虐待量表总得分、性虐待、躯体忽视和情感忽视得分分别为(36.89±10.84)、(6.00±1.79)、(7.84±3.11)、(9.58±5.57),均低于男双性恋者的(39.86±11.53)(t=2.524,P=0.012)、(6.62±2.76)(t=2.776,P=0.006)、(8.66±3.43)(t=2.401,P=0.017)、(11.02±6.45)(t=2.306,P=0.022)。多因素Logistic回归分析结果显示,儿童期虐待量表中情感虐待得分较高是MSM抑郁症状发生的危险因素,但未发现儿童期虐待对MSM抑郁症状发生的影响与性向有关。结论广州市男同/双性恋者的抑郁症状检出率较高,MSM抑郁症状的发生与儿童期虐待相关。今后应注重对MSM尤其是男同/双性恋者的心理干预。

Objective To investigate the depression status and its relationship with childhood trauma among men having sex with men(MSM)of homosexuality and bisexuality.Methods Convenient sampling method was used for an anonymous questionnaire survey among MSM who visited Yuexiu district center for disease control and prevention and Lingnan MSM partner support center for HIV testing and counseling services.Results A total of 424 MSM were surveyed,and 43.87%(186/424)had depression symptoms.The proportions of homosexuality and bisexuality among MSM were 70.52%(299/424)and 29.48%(125/424)respectively.The detection rates of depression symptoms among those with homosexual and bisexual behavior were 41.81%(125/299)and 48.80%(61/125),with no statistical difference.The total score of childhood trauma questionnaire(CTQ)(t=2.524,p=0.012),scores of childhood sexual abuse(t=2.776,p=0.006),physical neglect(t=2.401,p=0.017)and emotional neglect(t=2.306,p=0.022)were higher among the bisexual MSM.Multivariate logistic regression indicated that higher score of childhood emotional abuse was the risk factor related to depression among MSM.Conclusion The detection rates of depression symptoms among the MSM with homosexual and bisexual behavior are both high,and the occurrence of depression symptoms is related to higher score of childhood emotional abuse.Psychological intervention should be provided among MSM,especially those with both homosexual and bisexual behavior.
