后腹腔镜肾癌根治术对肾癌患者皮质醇、C反应蛋白及血肌酐的影响

Effect of retroperitoneal laparoscopic radical nephrectomy on cortisol,C-reactive protein and serum creatinine in patients with renal cell carcinoma

目的探讨肾癌患者应用后腹腔镜肾癌根治术治疗的临床疗效。方法选择肾癌患者120例,随机分为对照组和观察组,每组60例。对照组行开放性肾癌根治术,观察组行后腹腔镜肾癌根治术,比较2组手术创伤及恢复效果、并发症、皮质醇、C反应蛋白及血肌酐水平。结果观察组手术时间、出血量、恢复效果均优于对照组,差异有统计学意义(P<0.05);治疗后72 h,观察组皮质醇、C反应蛋白及血肌酐均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率11.67%,低于对照组的26.67%,差异有统计学意义(P<0.05)。结论肾癌患者行后腹腔镜肾癌根治术效果显著,可减少并发症发生,术后应激反应也较轻,肾功能恢复快。

Objective To investigate the clinical effect of retroperitoneal laparoscopic radical nephrectomy in treatment of patients with renal cell carcinoma.Methods Totally 120 patients with renal cell carcinoma were randomly divided into control group and observation group,with 60 cases in each group.The control group was treated with open radical nephrectomy,and the observation group was treated with retroperitoneal laparoscopic radical nephrectomy.The surgical trauma and recovery speed,complications,cortisol,C-reactive protein and serum creatinine levels were compared between the two groups.Results The operation time,blood loss and recovery effect of the observation group were significantly better than those of the control group(P<0.05).After 72 hours of treatment,the cortisol,C-reactive protein and serum creatinine levels in the observation group were significantly lower than those in the control group(P<0.05).The incidence of complications in the observation group was 11.67%,which was significantly lower than 26.67%in the control group(P<0.05).Conclusion Retroperitoneal laparoscopic radical nephrectomy is effective in treatment of patients with renal cell carcinoma,which can reduce the incidence of complications,and postoperative stress reaction is lighter,and the recovery of renal function is faster.
