血常规检查在地中海贫血、缺铁性贫血中的鉴别诊断价值

Differential Diagnosis Value of Blood Routine Examination in Thalassemia and Iron Deficiency Anemia

目的探讨血常规检查在地中海贫血、缺铁性贫血中的鉴别诊断价值。方法选取2017年5月至2019年5月于朝阳市中心医院接受治疗的90例贫血患者作为研究组,选取同期50名健康体检者作为对照组,两组均采用全自动血细胞分析仪检测血常规,比较两组红细胞计数(RBC)、血红蛋白(Hb)、平均红细胞比容(MCV)、红细胞血红蛋白含量(MCH)与浓度(MCHC)及红细胞体积分布宽度(RDW)等指标的差异。结果研究组地中海贫血36例、缺铁性贫血54例。地中海贫血组与缺铁性贫血组的RBC、Hb、MCV、MCH、MCHC及RDW指标与对照组比较,差异均有统计学意义(P<0.05);地中海贫血组、缺铁性贫血组RBC、Hb及MCHC水平与对照组比较,差异均有统计学意义(P<0.05);研究组与对照组MCV、MCH及RDW比较,差异均无统计学意义(P>0.05)。结论在血常规检查中,WBC、RBC、Hb及RDW水平用于地中海贫血与缺铁性贫血鉴别诊断的效果确切,具有显著的临床应用价值。

Objective To investigate the value of routine blood tests in the diagnosis and diff erential diagnosis of thalassemia and iron defi ciency anemia.Methods Selected 90 patients with anemia who were treated at Chaoyang Central Hospital from May 2017 to May 2019 as the research group,and selected 50 healthy subjects in the same period as the control group.The two groups were tested by automatic blood cell analyzer.The red blood cells(RBC),hemoglobin(Hb),mean hematocrit(MCV),red blood cell hemoglobin content(MCH)and concentration(MCHC)and red blood cell volume distribution width(RDW)and other indicators were compared.Results There were 36 cases of thalassemia and 54 cases of iron defi ciency anemia in the study group.The RBC,Hb,MCV,MCH,MCHC and RDW in the thalassemia group and iron defi ciency anemia group were signifi cantly diff erent from the control group(P<0.05).The levels of RBC,Hb and MCHC in the thalassemia group and iron defi ciency anemia group were signifi cantly diff erent from those in the control group(P<0.05).There was no signifi cant diff erence in MCV,MCH and RDW between the two groups(P>0.05).Conclusion The levels of WBC,RBC,Hb and RDW in blood routine examination are used for the screening and identifi cation of thalassemia and iron defi ciency anemia.The eff ect is exact and has signifi cant application value.
