摘要
目的分析产前超声检查在球拍状胎盘及帆状胎盘诊断中的临床应用价值,以期为球拍状胎盘、帆状胎盘诊断方法的选取提供参考。方法回顾性分析2017年7月至2019年6月于南阳市第一人民医院经产后病理证实为球拍状胎盘、帆状胎盘的112例患者的临床资料。入选者均接受产前超声检查。观察产前超声诊断球拍状胎盘、帆状胎盘结果,并以产后病理检查结果作为金标准,计算产前超声诊断球拍状胎盘及帆状胎盘的特异度、敏感度。结果经产后病理检查证实球拍状胎盘、帆状胎盘分别为70例、42例;70例球拍状胎盘中,经超声诊断66例,超声诊断球拍状胎盘敏感度、特异度分别为94.28%(66/70)、100.00%(42/42);42例帆状胎盘中,经超声诊断34例,超声诊断帆状胎盘敏感度、特异度分别为80.95%(34/42)、100.00%(70/70)。结论产前超声诊断球拍状胎盘及帆状胎盘准确率较高,利于观察脐带入口位置并及时发现异常,为临床诊断及妥善处理提供指导。
【Objective】To analyze the clinical value of prenatal ultrasound in the diagnosis of battledore placenta and velamentous placenta,so as to provide a reference for the selection of diagnosis methods.【Methods】The clinical data of 112 patients who were confirmed by pathology as battledore placenta and velamentous placenta in our hospital from July 2017 to June 2019 were analyzed retrospectively.All the participants received prenatal ultrasound examination.The results of prenatal ultrasound diagnosis of battledore placenta and velamentous placenta were observed,and the specificity and sensitivity of prenatal ultrasound diagnosis of battledore placenta and velamentous placenta were calculated.【Results】According to the pathological examination after delivery,there were 70 cases of battledore placenta and 42 cases of velamentous placenta.In 70 cases of battledore placenta,66 cases were diagnosed by ultrasound,and the sensitivity and specificity of ultrasound diagnosis for battledore placenta were 94.28%(66/70)and 100.00%(42/42).In 42 cases of velamentous placenta,34 cases were diagnosed by ultrasound,and the sensitivity and specificity of ultrasound diagnosis for velamentous placenta were 80.95%(34/42)and 100.00%(70/70).【Conclusion】The accuracy rate of prenatal ultrasound diagnosis is high,which is helpful to observe the entrance position of umbilical cord and find out the abnormality in time,and provide a guidance for clinical diagnosis and proper treatment.
