高危型HPV病毒载量及组织中miR-143表达与宫颈癌发病的关系研究

Relationship of incidence of cervical cancer with viral load of high-risk HPV and expression of miR-143 in tissues

目的探讨高危型HPV病毒(HR-HPV)载量及组织中miR-143表达与宫颈癌发病的关系,并分析其临床诊断价值。方法选取2016年12月至2019年12月本院诊治的220例宫颈病变患者为研究对象,其中宫颈上皮内瘤样病变Ⅰ级患者(CINⅠ组)52例、鳞状上皮中度非典型增生Ⅱ级患者(CINⅡ组)56例、鳞状上皮重度非典型增生Ⅲ级患者48例(CINⅢ组)及宫颈癌(CC组)患者64例;比较各组间组织miR-143的表达差异及高危型HPV病毒载量,并分析二者与宫颈癌临床病理之间的关系。结果HR-HPV载量水平方面,CINⅠ组<CINⅡ组<CINⅢ组<CC组,差异有统计学意义(P<0.05);miR-143表达水平方面,CINⅠ组>CINⅡ组>CINⅢ组>CC组,差异有统计学意义(P<0.05);宫颈癌患者HR-HPV载量和组织中miR-143表达水平与病理类型及淋巴结转移关系密切,差异均有统计学意义(均P<0.05);Pearson相关性结果显示,宫颈患者HR-HPV载量与组织中miR-143表达呈显著负相关(r=-0.655,P<0.05);ROC曲线分析显示,HR-HPV载量及组织中miR-143表达水平对宫颈癌的预测价值均较高,AUC分别为0.999、0.888。结论HR-HPV载量及组织中miR-143表达均与宫颈癌的发生密切相关,对宫颈癌均具较高预测价值。

Objective To explore the relationship of incidence of cervical cancer with viral load of high-risk HPV and expression of miR-143 in tissues.Methods Two hundred and twenty patients with cervical lesions diagnosed and treated at our hospital from December,2016 to December,2019 were selected as the research objects,including 52 patients with cervical intraepithelial neoplasia I(a CIN I group),56 patients with moderate atypical hyperplasiaⅡ(a CINⅡGroup),48 patients with severe atypical hyperplasiaⅢ(CINⅢgroup),and 64 patients with cervical cancer(a CC group).Differences in the expression of miR-143 and the viral load of HR-HPV were compared between the groups.The relationship between the two indicators and the clinicopathology of cervical cancer was analyzed.Results In terms of HR-HPV load level:the CIN I group<the CINⅡgroup<the CINⅢgroup<the CC group(all P<0.05).In terms of miR-143 expression level:the CIN I group>the CIN II group>the CIN III group>the CC group(all P<0.05).HR-HPV load and miR-143 expression in cervical cancer were closely related to pathological type and lymph node metastasis(all P<0.05).Pearson correlation results showed that HR-HPV load was negatively correlated with miR-143 expression(r=-0.655,P<0.05).ROC curve analysis showed that HR-HPV load and miR-143 expression level in tissues had high predictive values for cervical cancer(both P<0.05),and AUC's were 0.999 and 0.888,respectively.Conclusion The amount of HR-HPV and the expression of miR-143 in tissues are closely related to the occurrence of cervical cancer,which have high predictive valued for cervical cancer.
