基于脑肠轴理论的平胃戊己方治疗溃疡性结肠炎大肠湿热证的临床疗效

Evaluation of the clinical effect of Pingwei Wuji Decoction on Ulcerative Colitis due to Damp-heat Ayndrome of Large Intestine based on the Theory of Brain-gut Axis

目的探讨中药平胃戊己方联合美沙拉秦对溃疡性结肠炎大肠湿热证的临床疗效。方法将106例患者随机分为观察组与对照组,各53例。对照组予美沙拉秦,观察组在对照组的基础上给予中药平胃戊己方口服,疗程4周。比较两组临床疗效,中医症候评分、Mayo评分、Baron积分脑肠肽指标:血管活性肽(VIP)、P物质(SP)、5羟色胺(5-HT)、炎症细胞因子指标:肿瘤坏死因子-α(TNF-α)、白介素-8(IL-8)、白介素-10(IL-10)、生存质量IBDQ评分及6个月复发率。结果观察组总有效率显著优于对照组(P<0.05)。治疗后,2组中医症候评分、Mayo评分、Baron积分、SP、5-HT、TNF-α、IL-8均显著下降(P<0.05),VIP、IL-10、IBDQ评分明显升高(P<0.05),以观察组更明显(P<0.05)。观察组复发率显著优于对照组(P<0.05)。结论平胃戊己方联合美沙拉秦可改善溃疡性结肠炎大肠湿热证患者的临床症状,促进受损黏膜的修复,提高生存质量,降低复发率。

Objective To explore the clinical effect of Pingwei Wuji Decoction combined with mesalazine on patients with ulcerative colitis due to damp-heat syndrome of large Intestine. Methods 106 patients were randomly divided into control group(53 cases) for 4-week administration of Mesalazine, and observation group(53 cases) for 4-week administration of Pingwei Wuji Decoction and Mesalazine. Subsequently, the changes in clinical efficacy,TCM symptom score, Mayo score,Baron Scare,Brain gut peptide index: vasoactive peptide(VIP), substance P(SP), serotonin(5-HT), inflammatory cytokines index: tumor necrosis factor-α(TNF-α), interleukin-8(IL-8), interleukin-10(IL-10), quality of life IBDQ score and 6-month recurrence rate. Results The observation group demonstrated significantly higher total effective rate than the control group(P<0.05).After the treatment,TCM syndrome score,Mayo score,Baron Scare,SP, 5-HT, TNF-α and IL-8 in the observation group and the control group were significantly lower than those before treatment, and VIP, IL-10, IBDQ scores were significantly increased(P<0.05),especially for those in the observation group(P<0. 05).The recurrence rate of the observation group was significantly better than that of the control group(P<0. 05).Conclusion For the patients with ulcerative colitis due to damp-heat ayndrome of large intestine. Pingwei Wuji Decoction combined with mesalazine can improve the clinical symptoms of patients with ulcerative colitis due to damp-heat ayndrome of large intestine, promote the repair of damaged mucosa, improve the quality of life and reduce the recurrence rate.
