Value of three-dimensional speckle tracking technology in the early diagnosis of coronary heart disease

摘要 目的:分析三维斑点追踪成像(3D-STI)技术在冠心病早期诊断中的应用价值。方法:抽取2018年1月至2020年1月驻马店市中心医院收治的75例冠心病早期患者作为观察组,同时选取健康体检者75例作为对照组,均在静息条件下接受3D-STI检查。比较两组患者的心脏结构及功能指标。根据冠状动脉造影检查结果,将观察组进一步分为四组,比较各组左室收缩功能指标。结果:观察组患者的左室射血分数(LVEF)、每搏量(SV)低于对照组,收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)高于对照组,差异有统计学意义(P<0.05);与对照组比较,观察组整体圆周应变值(GCS)、整体径向应变值(GRS)、左室整体长轴应变值(GLS)、整体面积应变值(GAS)水平更低(P<0.05);观察组组内比较,单支+有侧支循环各项指标水平最高,多支+无侧支循环最低,差异有统计学意义(P<0.05)。结论:3D-STI可显示患者的心脏结构、功能改变及左室收缩功能情况,在冠心病早期诊断、病情评估中均有重要价值。 Objective To analyze the value of three-dimensional speckle tracking imaging(3D-STI)technology in the early diagnosis of coronary heart disease.Methods A total of 75 patients with early coronary heart disease in Zhumadian Central Hospital from January 2018 to January 2020 were selected as the observation group,while 75 healthy patients were selected as the control group.all patients underwent 3D-STI examination under resting conditions.The cardiac structure and function indexes of the two groups of patients were compared.According to the results of coronary angiography,the observation group was further divided into four groups,and the left ventricular systolic function indexes of each group were compared.Results The left ventricular ejection fraction(LVEF)and stroke volume(SV)of the observation group were significantly lower than those of the control group,while the end systolic volume(LVESV)and left ventricular end diastolic volume(LVEDV)of the observation group were significantly higher than those of the control group,the differences were statistically significant(P<0.05);compared with the control group,the observation group had lower levels of global circumferential strain(GCS),global radial strain(GRS),left ventricular global-axis strain(GLS),global area strain(GAS),P<0.05;in the comparison group,the single-branch+collateral circulation index was the highest,and the multi-branch+no collateral circulation was the lowest,the difference was significant(P<0.05).Conclusions 3D-STI can display the cardiac structure and function changes of patients and the left ventricular systolic function.It has important value in the early diagnosis and evaluation of coronary heart disease.
