经阴道超声对子宫腺肌症、子宫肌瘤和腺肌瘤的诊断价值

Diagnostic value of transvaginal ultrasound for adenomyosis,uterine fibroids and adenomyoma

目的探讨经阴道超声影像对子宫腺肌症、子宫肌瘤和腺肌瘤的诊断价值。方法选取2018年4月至2020年4月郑州人民医院收治子宫腺肌症、子宫肌瘤和腺肌瘤患者各50例,均经由手术病理得到证实。分别对其进行经阴道彩色多普勒超声检查和经腹部超声检查,对两种检查方式的诊断结果进行对比分析。结果经阴道超声对子宫肌瘤、子宫腺肌症、子宫腺肌瘤患者的诊断准确率均明显高于经腹部超声,差异有统计学意义(P <0.05);经腹部超声对子宫腺肌症、子宫肌瘤的诊断准确率明显高于子宫腺肌症,差异有统计学意义(P <0.05);子宫肌瘤患者的RI、PI、Vd、Vs均明显低于子宫腺肌症、腺肌瘤患者;子宫腺肌瘤患者的RI、Vs明显高于其他两组,子宫腺肌症患者的Vs明显高于其他两组,差异有统计学意义(P <0.05)。结论经阴道超声对子宫肌瘤、子宫腺肌症、子宫腺肌瘤三种子宫病变的诊断比经腹部超声具有显著优势,同时结合彩色多普勒超声血流信号特征,可有效提高对于三种病变的鉴别诊断准确性。

Objective To investigate the diagnostic value of transvaginal ultrasound imaging for adenomyosis,uterine fibroids and adenomyoma. Methods From April 2018 to April 2020,50 patients with adenomyosis,50 patients with uterine fibroids and 50 patients with adenomyoma were admitted to the People’s Hospital of Zhengzhou,all of which were confirmed by surgical pathology. Transvaginal color Doppler ultrasound examination and transabdominal ultrasound examination were performed respectively,and the diagnosis results of the two examination methods were compared and analyzed. Results The diagnostic accuracy of transvaginal ultrasound for uterine fibroids,adenomyosis,and adenomyoma was significantly higher than that of transabdominal ultrasound,the differences were significant( P < 0. 05).In transabdominal ultrasound,the diagnostic accuracy of adenomyosis and hysteromyoma was significantly higher than that of adenomyosis,and the differences were significant( P < 0. 05). The levels of RI,PI,Vd,Vs of uterine fibroids were significantly lower than those of adenomyosis patients with adenomyosis and adenomyoma. The levels of RI and Vs of patients with adenomyoma were significantly higher than those of the other two groups,and the level of Vs of patients with adenomyosis were significantly higher than that of the other two groups,the differences were significant( P < 0. 05). Conclusions For patients with uterine fibroids,adenomyosis,and adenomyoma,transvaginal ultrasound has significant advantage in diagnosis compared with transabdominal ultrasound, while combining color Doppler ultrasound blood flow signal characteristics,can effectively improve the accuracy of differential diagnosis of the three types of lesions.
