右美托咪定在女性腹腔镜患者手术中的应用

Application of Dexmedetomidine in Female Patients With Laparoscopic Surgery

目的探讨围术期使用右美脱咪定对女性腹腔镜手术患者的效果。方法随机筛选2018年6月—2019年12月在本院接受腹腔镜手术的60例女性患者,按照双盲法将其分为观察组(n=30)和对照组(n=30),均行气管插管全身麻醉。观察组在全麻诱导前、全麻过程中及术后予以右美托咪定泵入,对照组予以等量生理盐水。观察两组患者术后48 h内恶心呕吐(postoperative nausea and vomiting,PONV)情况及疼痛、心动过缓等不良反应的发生情况。结果两组患者术后恶心呕吐发生情况在0~6 h和7~24 h比较,观察组优于对照组,差异有统计学意义(P <0.05),两组25~48 h比较差异无统计学意义(P>0.05)。观察组术后疼痛情况优于对照组(P <0.05),两组心动过缓发生情况比较差异无统计学意义(P> 0.05)。结论右美托咪定能够降低腹腔镜术后恶心呕吐严重程度,减少术后抗呕吐药物的使用,并有助于改善术后疼痛。

Objective To investigate the effect of perioperative use of dexmedetomidine on female patients undergoing laparoscopic surgery.Methods Randomly screened 60 female patients who underwent laparoscopic surgery in our hospital from June 2018 to December 2019,and they were divided into the observation group(n=30)and the control group(n=30)according to the double-blind method.All of them used general anesthesia for tracheal intubation.The observation group was given dexmedetomidine pump before general anesthesia induction,during and after general anesthesia,and the control group was given the same amount of normal saline.The occurrence of adverse reactions such as postoperative nausea and vomiting(PONV),pain and bradycardia were observed in the two groups of patients in 48 hours after operation.Results The postoperative nausea and vomiting of the two groups were compared between 0-6 hours and 7-24 hours.The observation group was better than the control group.The difference was statistically significant(P<0.05).There was no statistical difference between the two groups at 25-48 hours(P>0.05).The postoperative pain of the observation group was better than that of the control group(P<0.05),and there was no difference in bradycardia(P>0.05).Conclusion Dexmedetomidine can reduce the severity of postoperative nausea and vomiting after laparoscopic surgery,reduce the use of postoperative antiemetic drugs,and help improve postoperative pain.
