摘要
目的:探讨长链非编码RNA前列腺癌基因表达标记物1(lncRNA PCGEM1)在早期原发性肝细胞癌患者外周血中的表达及其对诊断和预后的意义。方法:选取2016年3月—2019年12月在宣城市中心医院普外科接受肝癌切除术治疗的70例早期原发性肝细胞癌患者,其中男性62例,女性8例;年龄31~71岁,平均(49.04±9.26)岁。另外选择同期体检的70名健康成年志愿者作为对照组,其中男性61例,女性9例;年龄25~68岁,平均(49.40±8.86)岁。采用qRT-PCR检测患者手术前后和健康体检者外周血中PCGEM1的表达;分析其与早期肝癌临床病理特征间的关系。应用受试者工作特征(ROC)曲线分析PCGEM1对早期肝癌诊断和肝癌细胞分级预测的价值。结果:患者术前外周血中PCGEM1的表达量高于术后和健康体检者(均P<0.001),ROC分析结果显示血清PCGEM1能对早期肝癌诊断和肝癌细胞分级进行预测(均P<0.001)。肿瘤直径>5 cm、无肿瘤包膜和肝癌细胞分级差(Ⅲ级和Ⅳ级)的早期肝癌患者血清PCGEM1的表达量升高(均P<0.05)。结论:早期原发性肝细胞癌患者外周血中PCGEM1的表达升高,高表达患者肿瘤直径大且恶性程度高,提示预后较差。
Objective:To investigate the expression of lncRNA prostate cancer gene expression marker 1(PCGEM1)in the peripheral blood of patients with early primary hepatocellular carcinoma and its significance in diagnosis and prognosis.Methods:70 patients with early primary hepatocellular carcinoma who underwent hepatocellular carcinoma resection at the General Surgery Department of Xuancheng Central Hospital from March 2016 to December 2019 were selected,including 62 males and 8 females;ages 31-71,average(49.04±9.26)years old.In addition,70 healthy adult volunteers who received physical examination during the same period were selected as the control group.Among them,61 were males and 9 were females;they were 25-68 years old,with an average of(49.40±8.86)years old.qRT-PCR was used to detect the expression of PCGEM1 in the peripheral blood of patients before and after surgery and in healthy subjects;the relationship between it and the clinicopathological characteristics of early liver cancer was analyzed.The receiver operating characteristic(ROC)curve was used to analyze the value of PCGEM1 in the early diagnosis of liver cancer and the prediction of liver cancer grading.Results:The expression of PCGEM1 in the peripheral blood of patients before operation was higher than that in postoperative and healthy subjects(both P<0.001).ROC analysis showed that serum PCGEM1 could predict early liver cancer diagnosis and liver cancer grading(all P<0.001).The expression of serum PCGEM1 in early liver cancer patients with tumor diameter>5 cm,no tumor envelope and poor grades of liver cancer cells(gradeⅢandⅣ)was increased(all P<0.05).Conclusion:The expression of PCGEM1 in the peripheral blood of patients with early primary hepatocellular carcinoma is elevated.Patients with high expression have large tumor diameters and high malignancy,suggesting a poor prognosis.
