奥马哈系统干预在老年膀胱肿瘤患者术后膀胱灌注化疗中的应用

Application of Omaha SystemIntervention in Postoperative Bladder Perfusion Chemotherapy in Elderly Patients with Bladder Cancer

目的探讨奥马哈系统干预在老年膀胱肿瘤患者术后膀胱灌注化疗中的应用效果。方法选取2018年1月-2019年6月于儋州市人民医院泌尿外科住院治疗的老年膀胱癌术后患者120例,按照随机数字表法将患者分为观察组和对照组,每组60例。所有患者接受术后膀胱灌注化疗,给予对照组患者实施常规干预,观察组患者使用奥马哈系统进行干预,比较2组患者干预后膀胱灌注情况、膀胱灌注依从性、自我管理效能及生活质量。结果观察组患者按时灌注比例显著高于对照组(P<0.01);患者的肿瘤知识、灌注知识、不良反应处理及定期复查评分均显著高于对照组(P<0.05);观察组的正性态度、自我决策、自我减压评分及自我管理效能总分均显著高于对照组(P<0.05);观察组生活质量评分(QOL)中的躯体领域、心理领域、社会领域的评分和总分均显著高于对照组(P<0.05)。结论奥马哈系统干预可改善老年膀胱肿瘤患者术后膀胱灌注化疗情况,提高其依从性、自我管理效能及生活质量。

Objective To investigate the effect of Omaha system intervention on postoperative bladder perfusion chemotherapy in elderly patients with bladder cancer.Methods A total of 120 elderly patients with bladder cancer who were hospitalized at the Department of Urology of People's Hospital of Danzhou City fromJanuary 2018 to June 2019 were selected.According to simple random sampling,they were divided into observation group(n=60)and control group(n=60).All patients received postoperative bladder perfusion chemotherapy.Meanwhile,the control group received routine intervention and the observation group received Omaha systemintervention.The bladder perfusion condition,bladder perfusion compliance,self-management efficiency and life quality of patients after intervention were compared between the two groups.Results The proportion of patients with timely perfusion in the observation group was significantly higher than that in the control group(P<0.01);the tumor knowledge,perfusion knowledge,adverse reaction management and regular review scores of the observation group were significantly higher than those of the control group(P<0.05);the positive attitude,self-decision,selfdecompression score and total score of self-management efficacy of the observation group were significantly higher than those of the control group(P<0.05).The scores of the physical,psychological,and social fields in the quality of life score(QOL)and the total QOL scores of the observation group were significantly higher than those of the control group(P<0.05).Conclusion Omaha system intervention can improve the postoperative bladder perfusion chemotherapy in elderly patients with bladder cancer,and improve their compliance,self-management efficiency and quality of life.
