医院内镜日间病房老年患者就医体验及其影响因素分析

Analysis of Medical Experience of Elderly Patients in Endoscopic Day Ward and Its Influencing Factors

目的调查内镜日间病房老年患者就医体验,分析就医体验的影响因素,为改善老年患者就医体验,提高医院医疗服务水平提供依据。方法采取整群抽样法,选择2020年10月20日-2020年11月9日入住复旦大学附属中山医院内镜日间病房老年患者为研究对象,以住院患者体验量表(IPEQ)调查患者的就医体验,选出得分最高前5项与得分最低前5项,并依据IPEQ总得分结果分为就医体验满意组(IPEQ评分≥88分)和就医体验不满意组(<88分),比较2组临床资料,分析医院内镜日间病房老年患者就医体验的影响因素。结果本次调查的老年患者平均就医体验得分为120.77分,总体满意平均分为96.32分。在核心体验的7个维度评分中,技术质量体验得分最高(平均4.65分),情感支持体验得分最低(平均3.68分);在29个单项体验条目中,A22平均分最高(4.76分),而A11平均分最低(3.23分),A4、A10评分也较低(分别为3.46分和3.48分),其他项条目平均得分在3.6~4.7分。单因素及多因素logistic回归分析发现,文化程度低、就诊前对日间手术不了解及A4、A10、A11得分低为影响医院内镜日间病房老年患者就医体验不满意的独立影响因素(P<0.05)。结论内镜日间病房患者总体就医体验较好,在情感支持、服务态度、可及便利度及疾病交流方面有待提升。

Objective To investigate the medical experience of elderly patients in the endoscopic day ward of a hospital and analyze its influencing factors,so as to provide a basis for improvement of medical experience of elderly patients and the level of hospital medical services.Methods The elderly patients who were admitted to the Endoscopic DayWard of Zhongshan Hospital Affiliated to Fudan University from October 20,2020 to November 9,2020 were selected as the subjects by cluster sampling method.The Inpatient Experience Scale(IPEQ)was used to investigate the medical experience of the subjects,and the top 5 items with the highest score and the top 5 items with the lowest score were selected.According to the total IPEQ score,the subjects were divided into satisfactory group(IPEQscore≥88 points)and unsatisfactory group(IPEQ score<88 points).Clinical data of the two groups were compared,and factors affecting the medical experience of elderly patients in endoscopic day ward were analyzed.Results The average medical experience score of the subjects in this survey was 120.77 points,and the average overall satisfaction score was 96.32 points.Among the 7 dimensional scores of core experience,the technical quality experience score was the highest(average 4.65 points),and the emotional support experience score was the lowest(average 3.68 points).Among the 29 single experience items,the average score of A22 was the highest(4.76),while the average score of A11 was the lowest(3.23),the score of A4 and A10 was also low(3.46 and 3.48,respectively),and the average score of other items ranged from 3.6 to 4.7 points.Univariate and multivariate logistic regression analysis showed that low education level,lack of understanding of day surgery before treatment and low scores of A4,A10 and A11 were the independent influencing factors of unsatisfactory medical experience of elderly patients in endoscopic day ward(P<0.05).Conclusion Patients in the endoscopic day ward have a good overall medical experience,but the endoscopic day ward needs to be improved in terms of emotional support,service attitude,accessibility and disease knowledge communication.
