家庭康复管理对急性缺血性脑卒中患者认知功能、运动功能和生存质量的影响

Effect of family rehabilitation management on cognitive function,motor function,quality of life of patients with acute ischemic stroke

目的分析家庭康复管理对急性缺血性脑卒中患者认知功能、运动功能和生存质量的影响。方法选取2018年3月至2020年3月中国人民解放军东部战区空军医院和中国人民解放军东部战区总医院秦淮医疗区收治的226例急性缺血性脑卒中患者为研究对象,使用随机数字表法将入选患者分为研究组和对照组,每组各113例,均予常规药物治疗和康复干预,研究组患者在此基础上接受家庭康复管理,根据家庭照顾者经历的混乱期、面对期、学习共处期和超越期4个阶段对急性缺血性脑卒中患者开展家庭康复管理。比较两组患者干预前和干预3个月后的认知功能、运动功能和生存质量。结果两组患者干预3个月后简易精神状态检查评分、Fugl-Meyer评定量表评分、Barthel指数评分、生存质量各领域评分及总分均显著高于本组干预前(均P<0.05),研究组患者干预3个月后上述指标评分均显著高于对照组(均P<0.05)。结论在常规药物治疗和康复干预的基础上,根据家庭照顾者经历的不同时期开展家庭康复管理,能够改善急性缺血性脑卒中患者的认知功能、运动功能和生存质量。

Objective To analyze the effect of family rehabilitation management on cognitive function,motor function,quality of life of patients with acute ischemic stroke.Method From March 2018 to March 2020,226 patients with acute ischemic stroke admitted to PLA Eastern Theater Air Force Hospital and Qinhuai Medical Area of Eastern Theater General Hospital of PLA were selected and randomly divided into study group and control group,with 113 cases in each group.All patients were given conventional drugs treatment and rehabilitation intervention.Study group patients received additional family rehabilitation management.Family rehabilitation management should be carried out according to four stages:chaotic period,facing period,study coexistence period and overstepping period experienced by family caregivers.The cognitive function,motor function and quality of life of patients were compared between the two groups before and 3 months after intervention.Result The scores of mini-mental state examination,Fugl-Meyer assessment scale,Barthel index,and various fields of quality of life as well as the total scores in the two groups 3 months after intervention were significantly higher than those before intervention in the same group(all P<0.05).The scores of the above indexes in study group 3 months after intervention were all significantly higher than those in control group(all P<0.05).Conclusion On the basis of conventional drugs treatment and rehabilitation intervention,family rehabilitation management according to the different periods experience by family caregivers can improve the cognition function,motor function and quality of life of patients with acute ischemic stroke.
