多节段椎弓根钉置入术治疗强直性脊柱炎合并脊柱骨折的临床效果

Clinical effect of multi segmental pedicle screw placement in the treatment of ankylosing spondylitis with spinal fracture

目的探讨多节段椎弓根钉置入术治疗强直性脊柱炎(AS)合并脊柱骨折的临床效果。方法将我院收治的80例AS合并脊柱骨折患者分为对照组(40例,切开复位内固定术)和观察组(40例,多节段椎弓根钉置入术)。比较两组治疗效果。结果观察组的治疗优良率高于对照组(P<0.05)。术后1个月,观察组的Frankel分级、VAS评分均优于对照组(P<0.05)。术后1周及6个月,两组伤椎Cobb角、伤椎前缘高度百分比均较术前改善,且观察组术后1周的伤椎Cobb角、伤椎前缘高度百分比均优于对照组(P<0.05)。术后,观察组的各项JOA评分及总分均高于对照组(P<0.05)。结论多节段椎弓根钉置入术治疗AS合并脊柱骨折的临床效果显著,创伤小、疼痛轻,还可促进脊髓神经、腰背功能恢复。

Objective To investigate the clinical effect of multi segmental pedicle screw placement in the treatment of ankylosing spondylitis(AS)with spinal fracture.Methods Eighty patients with AS and spinal fracture admitted in our hospital were divided into control group(40 cases,open reduction and internal fixation)and observation group(40 cases,multi segmental pedicle screw placement).The treatment effect of the two groups was compared.Results The excellent and good rate of the observation group was higher than that of the control group(P<0.05).One month after operation,the Frankel grade and VAS score of the observation group were better than those of the control group(P<0.05).At 1 week and6 months after operation,Cobb angle and anterior height percentage of injured vertebrae in two groups improved compared with those before operation,and at 1 week after operation,Cobb angle and anterior height percentage of injured vertebrae in observation group were better than those in control group(P<0.05).After operation,the JOA score and total score of the observation group were higher than those of the control group(P<0.05).Conclusion Multi segmental pedicle screw placement in the treatment of AS with spinal fracture has a significant clinical effect,with less trauma and pain,which can promote the recovery of spinal nerves and lower back function.
