关于达芬奇机器人和腹腔镜应用于直肠癌根治术的临床疗效对比研究

Comparative Study on The Clinical Efficacy of Da Vinci Robot and Laparoscopy in RadicalResection of Rectal Cancer

目的通过本次研究明确直肠癌根治手术中,达芬奇机器人和腹腔镜哪一种方式的应用优势更明显。方法将95例直肠癌患者随机分为两个组别,观察1组共48例,观察2组共47例,两组分别接受基于达芬奇机器人和腹腔镜的直肠癌根治术,选取观察指标,进行两组的临床疗效评价和组间对比。结果先对比两组术中和术后的部分指标,结果显示观察1组术中清扫淋巴结数量较观察2组多(P<0.05),术后拔除尿管时间、恢复正常饮食时间均较观察2组早(P<0.05)。术后并发症发生率这一指标,观察1组为4.17%,观察2组为6.38%,组间差异不明显(P>0.05)。两组术前的肿瘤患者生活质量评定量表(QOL)评分比较,组间差异不明显(P>0.05),观察1组术后1周、术后2周的QOL评分均较观察2组高(P<0.05)。结论无论是腹腔镜辅助还是达芬奇机器人辅助下的直肠癌根治术,在治疗安全性上的差异并不明显,而在淋巴结清扫效果方面,后者的效果更彻底,且术后恢复更快。

Objective Through this study to determine the radical surgery for rectal cancer,Da Vinci robot and laparoscopic which method of application advantage is more obvious.Methods A total of 95 patients with rectal cancer were randomly divided into two groups:observation group(48 cases in total)and observation group(47 cases in total).The two groups received radical resection of rectal cancer based on Da Vinci robot and laparoscope respectively.Observation indexes were selected for clinical efficacy evaluation and intergroup comparison.Results Firstly,some indexes of the two groups were compared during and after operation.The results showed that the number of lymph nodes dissected during operation in the observation group was more than that in the observation group(P<0.05),and the time of removing urinary tube and restoring normal diet after operation was earlier than that in the observation group(P<0.05).Postoperative complication rate was 4.17%in the observation group and 6.38%in the observation group,with no significant difference between the two groups(P>0.05).Comparison of preoperative quality of life(QOL)scores between the two groups showed no significant difference(P>0.05),and the QOL scores of the observation group 1 week and 2 weeks after surgery were higher than those of the observation group 2(P<0.05).Conclusion There was no significant difference in the safety of laparoscopic or Da Vinci robot-assisted rectal cancer radical resection,while the latter showed more thorough efficacy and faster postoperative recovery in terms of lymph node dissection.
