比较三维肛肠超声与X线排粪造影对直肠前突(RC)及其合并症的临床诊断价值

Objective to Compare the Clinical Value of Three-Dimensional Anorectal Ultrasound andX-ray Defecography in the Diagnosis of Rectocele (RC) and Its Complications

目的分析三维肛肠超声与X线排粪造影对直肠前突及其合并症的临床诊断价值。方法采用回顾性分析的形式记录本院于2016年11月至2019年3月间接收的直肠前突患者共计36例,对所有患者均予以三维肛肠超声与X线排粪造影检查,对比两种诊断形式的一致性以及合并症状的检出率和相关性。结果三维肛肠超声共计检查诊断直肠前突患者28例,其中14例合并膀胱脱垂、10例合并盆底失弛缓、4例子宫脱垂、4例直肠粘膜内套叠、2例肠疝。X线排粪造影共计检查直肠前突患者36例,其中24例合并膀胱脱垂、16例合并盆底失弛缓、16例合并子宫脱垂、12例合并直肠粘膜内套叠、2例肠疝;三维肛肠超声和X线排粪造影检查,测定直肠前突深度之间的差异性和相关性明显(P<0.05)。结论三维肛肠超声与X线排粪造影检查直肠前突的一致性高,两者测定的直肠前突深度存在较高的差异性和相关性,但是相对来说,三维肛肠超声诊断较快,患者具有更好的耐受性。

Objective To analyze the clinical value of three-dimensional anorectal ultrasound and X-ray defecography in the diagnosis of rectocele(RC)and its complications.Methods A total of 36 patients with rectocele received in our hospital from November 2016 to March 2019 were retrospectively analyzed.All patients were examined by three-dimensional anorectal ultrasound and X-ray defecography,and the consistency of the two diagnostic forms and the detection rate and correlation of the combined symptoms were compared.Results 28 patients with rectocele were diagnosed by three-dimensional anorectal ultrasound,including 14 cases with bladder prolapse,10 cases with pelvic floor achalasia,4 cases with uterine prolapse,4 cases with rectal mucosal intussusception and 2 cases with intestinal hernia.Thirty six patients with rectocele were examined by X-ray defecography,including 24 cases with bladder prolapse,16 cases with pelvic floor achalasia,16 cases with uterine prolapse,12 cases with rectal mucosal intussusception,and 2 cases with intestinal hernia.The difference and correlation between three-dimensional anorectal ultrasound and X-ray defecography were significant(P<0.05).Conclusion Three dimensional anorectal ultrasound and X-ray defecography examination of rectal protrusion have high consistency,and there is a high difference and correlation between them.But relatively speaking,three-dimensional anorectal ultrasound diagnosis is faster and patients have better tolerance.
