壮筋续骨方联合改良内固定术及康复训练治疗中青年股骨颈骨折的效果观察

Effect of Zhuangjinxuegu Recipe combined with improved internal fixation and rehabilitation training on young and middle-aged patients with femoral neck fractures

目的探讨壮筋续骨方联合改良内固定术及康复训练治疗中青年股骨颈骨折的临床效果。方法选择2017年5月至2019年5月收治的青年股骨颈骨折患者60例,随机分为观察组和对照组各30例,观察组给予壮筋续骨方联合改良内固定术及康复训练治疗,对照组给予改良内固定术联合康复训练治疗,比较两组术后髋关节功能、术后髋关节Harris评分、术后SF-36量表评分、骨折愈合时间及并发症发生情况。结果观察组患者术后髋关节功能优良率较对照组高,术后髋关节Harris评分系统中观察组患者功能、疼痛、活动度评分较对照组高,差异均具有统计学意义(P<0.05),两组患者畸形评分差异无统计学意义(P>0.05),术后SF-36量表中观察组患者整体健康、社交功能、活力、情绪角色、心理健康、躯体疼痛、躯体功能、躯体角色评分较对照组高,观察组患者骨折愈合时间较对照组短,股骨头坏死发生率较对照组低,差异均具有统计学意义(P<0.05),两组患者骨塌陷和骨折不愈合发生率差异无统计学意义(P>0.05)。结论壮筋续骨方联合改良内固定术及康复训练治疗中青年股骨颈骨折可促进骨折愈合,改善髋关节功能,提高生活质量,降低股骨头坏死发生率,值得在临床上推广。

Objective To analyze the clinical effects of young and middle-aged femoral neck fractures treated with Zhuangjinxuegufang combined with improved internal fixation and rehabilitation training.Methods Sixty young patients with femoral neck fractures who were treated from May 2017 to May 2019 were randomly divided into the observation group and the control group,with 30 cases in each group.The observation group was given Zhuangjinxugu Recipe combined with improved internal fixation and rehabilitation training.The control group was given modified internal fixation combined with rehabilitation training.The hip function,Harris score,SF-36 scale score,fracture healing time and complications of the two groups were compared.Results The rate of excellent and good rate of postoperative hip joint function in the observation group was significantly higher than that in the control group.The function,pain,and activity scores of postoperative hip joint Harris score system in the observation group were higher than those of the control group.The differences were statistically significant(P<0.05),there was no significant difference in deformity score between the two groups(P>0.05).The overall health,social function,vitality,emotional role,mental health,physical pain,physical function and body role scores of SF-36 were significantly higher than those in the control group.The fracture healing time in the observation group was significantly shorter than that in the control group.The incidence of femoral head necrosis was lower than that of the control group.The differences were statistically significant(P<0.05).There was no significant difference in the incidences of bone collapse and fracture nonunion between the two groups(P>0.05).Conclusion Zhuangjinxuegu Recipe combined with improve internal fixation and rehabilitation training for young and middle-aged femoral neck fractures can promote fracture healing,improve hip function,improve quality of life,and reduce the incidence of femoral head necrosis,which is worthy of clinical promotion.
