化脓性阑尾炎单孔腹腔镜术后效果观察

Effects of single-port laparoscopy on patients with suppurative appendicitis

目的探讨单孔腹腔镜手术治疗化脓性阑尾炎患者的效果。方法选择2018年2月至2020年3月本院收治的化脓性阑尾炎患者82例,对照组40例采用开腹手术,观察组42例采用单孔腹腔镜。对比两组手术指标、术后胃肠功能、炎症相关指标变化及并发症。结果观察组首次肛门排气及排便时间、肠鸣音恢复时间较对照组短(P<0.05);术后1d、3d、5d,观察组IL-6、CRP水平均较对照组低(P<0.05);观察组尿路感染、切口感染、肠瘘、肠梗阻等并发症发生率较对照组低(P<0.05)。结论化脓性阑尾炎患者采用单孔腹腔镜手术治疗,对机体炎症反应影响较小,可加快胃肠功能恢复,降低并发症发生,安全性较高。

Objective To investigate the effect of single-port laparoscopy on patients with suppurative appendicitis.Methods A total of 82 patients with suppurative appendicitis who were treated in our hospital from February 2018 to March 2020 were selected.Forty patients who underwent open surgery were included in the control group,and 42 patients who underwent single-port laparoscopy were included in the observation group.The operation indexes,postoperative gastrointestinal function,inflammation-related index changes and complications were compared between the two groups.Results The time of first anal exhaustion,bowel movement and recovery of bowel sounds in the observation group were shorter than those in the control group(P<0.05).The levels of IL-6 and CRP in the observation group were lower than those in the control group at 1d,3d,and 5d after operation(P<0.05).The incidence of complications such as urinary tract infection,incision infection,intestinal fistula,and intestinal obstruction in the observation group was lower than that in the control group(P<0.05).Conclusion Patients with suppurative appendicitis who use the single-hole laparoscopic surgery for treatment have little effect on the body’s inflammatory response,which can accelerate the recovery of gastrointestinal function,reduce the occurrence of complications,and have higher safety.
