基于数据挖掘技术分析心力衰竭中医方剂的用药规律

Rules of Traditional Chinese Medicine Prescription in the Treatment of Heart Failure Based on Data Mining Methods

目的运用中医传承辅助系统(V2.5)分析心力衰竭中医方剂的用药规律。方法在中国期刊全文数据库(CNKI)中以主题词为“心衰OR心力衰竭”AND主题词为“中医OR中医药OR中西医结合”检索2017年1月—2020年1月发表的文献,从中选取与中医药治疗相关的临床研究文献,并运用中医传承辅助平台软件对药物频次、四气、五味、归经、药物组合及关联规则进行分析。结果共筛选出327篇符合标准的文献,涉及179味中药,其中使用频次≥20次的药物共38味;药性方面,排在首位的是温,其次为寒、平;药味方面,排在前3位的是甘、苦、辛。归经方面,排在前5位的是脾、心、肺、肝、肾;当支持度个数为65时,共得到28个药物组合;当置信度为0.8时,共得到4个关联规则。结论心力衰竭的主要病机为气虚血瘀水停,治疗多以补益正气、利水渗湿、活血化瘀为法,临床上多以补气药、利水药、活血化瘀药配伍使用。

Objective To investigate the rules of traditional Chinese medicine prescription in the treatment of heart failure based on Traditional Chinese Medicine Inheritance Auxiliary Platform.Methods Journal Full-Text Database(CNKI),the subjects of"heart failure"and"traditional Chinese medicine or integrated traditional Chinese and western medicine"were used as keywords to search the literatures from January 2017 to January 2020.Clinical research literatures related to the treatment of traditional Chinese medicine were selected,and drug frequency,siqi wuwei,drug combination,and association rules were analyzed by Traditional Chinese Medicine Inheritance Auxiliary Platform.Results A total of 327 qualified literatures and 179 traditional Chinese medicines were included in this study.Among them,38 drugs were used more than 20 times.In terms of medicinal properties,worm ranked the first,followed by cold and flat.In terms of medicinal taste,the top three were gan,ku and xin.In the aspect of meridian arrangement,the top five were spleen,heart,lung,liver and kidney.When the number of support degrees was 65,a total of 28 drug combinations were obtained.When the confidence was 0.8,a total of four association rules were obtained.Conclusion The main pathogenesis of heart failure was qi deficiency,blood stasis and water stasis.The treatment was mainly based on the methods of supplementing qi,benefiting water infiltration and dampness,promoting blood circulation and removing blood stasis.The clinical practice was mostly based on the compatibility of supplementing qi drugs,benefiting water infiltration and dampness,promoting blood circulation and removing blood stasis.
