灯盏花素注射液联合清肺化痰汤治疗慢性阻塞性肺疾病急性加重期临床研究

Clinical Study on Breviscapine Injection Combined with Qingfei Huatan Tang for AECOPD

目的:观察灯盏花素注射液联合清肺化痰汤治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床疗效。方法:将AECOPD患者126例随机分为2组,各63例。对照组在西医常规治疗的基础上加用灯盏花素注射液治疗,观察组在对照组的基础上加用清肺化痰汤治疗。疗程均为2周,观察比较2组治疗前后中医证候积分、肺功能[第1秒钟用力呼气容积(FEV1)、FEV1/用力肺活量(FVC)]、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6 (IL-6)、白细胞介素-8 (IL-8)]水平变化。结果:治疗后,2组患者咳嗽、咳痰、胸闷等证候评分均较治疗前明显降低(P<0.05),且观察组各项评分均明显低于对照组(P<0.05)。治疗后,2组FEV1、FEV1/FVC均较治疗前明显升高(P<0.05),且观察组2项指标均明显高于对照组(P<0.05)。治疗后,2组血清TNF-α、IL-6、IL-8水平较治疗前明显降低(P<0.05),且观察组各项指标均明显低于对照组(P<0.05)。结论:灯盏花素注射液联合清肺化痰汤治疗AECOPD可有效控制肺部炎症反应,改善咳痰咳嗽症状,促进肺功能恢复。

Objective:To observe the clinical effect of breviscapine injection combined with Qingfei Huatan tang for acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods:A total of 126 cases of AECOPD patients were randomly divided into two groups,63 cases in each group. The control group was treated with breviscapine injection based on routine western medicine, and the observation group was additionally treated with Qingfei Huatan tang based on the treatment of the control group. Both groups were treated for two weeks. Before and after treatment,the changes in Chinese medicine syndrome scores, lung function including forced expiratory volume in one second(FEV1) and FEV1/forced vital capacity(FVC) and levels of inflammatory factors including tumor necrosis factor α(TNF-α), interleukin-6(IL-6) and interleukin-8(IL-8) in the two groups were observed and compared. Results: After treatment, the scores of coughing,expectoration, chest distress and other syndromes in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the scores in the observation group were significantly lower than those in the control group(P<0.05). After treatment, FEV1 and FEV1/FVC in the two groups were significantly increased when compared with those before treatment(P<0.05),and the two indexes in the observation group were significantly higher than those in the control group(P<0.05). After treatment,the levels of TNF-α,IL-6 and IL-8 in serum in the two groups were significantly decreased when compared with those before treatment(P<0.05), and the above indexes in the observation group were significantly lower than those in the control group(P<0.05). Conclusion:Breviscapine injection combined with Qingfei Huatan tang for AECOPD can effectively control pulmonary inflammation, improve expectoration and coughing, and promote the recovery of lung function.
